研究背景:發炎指標以被證實為憂鬱症的風險原因之一,且發炎指標愈多復發機率愈高。研究指出發炎因子與憂鬱症之間存在的關聯,但缺乏屬於精神疾患的獨特性發炎指標,因此在臨床上值得被更好地瞭解憂鬱症與發炎指標之相關性預測。
研究目的:探討憂鬱症病患之炎症指標與憂鬱症復發之相關性。
研究設計:本研究資料來源為北醫大臨床資料庫,來自台北醫大學附設醫院、臺北市立萬芳醫院、新北市部立雙和醫院被診斷為憂鬱症且針對單次發作、復發的個案進行分析。
研究方法:北醫大臨床研究資料庫,包含臺北醫學大學附設醫院、臺北市立萬芳醫院、新北市部立雙和醫院經營的三家體系,自2004年起含蓋體系醫院各類的醫療電子紀錄,內容包含:性別、年齡、職業、教育程度等人口學特徵;臨床抽血檢驗、用藥情形、入院原因、診斷、住院次數等就診之醫療紀錄。
本研究將利用北醫大臨床資料庫進行憂鬱症病患之回溯性研究,針對資料庫用診斷碼搜集憂鬱症個案以及ICD-9、ICD-10來篩選出憂鬱症單次及憂鬱症復發並有抽血發炎指數之個案進行分析,並運用統計軟體SPSS 26.0版套裝軟體進行資料整合與分析。
研究結論:單次憂鬱症與復發在藥物使用與發炎指標之差異,WBC數值於復發組的病患中明顯高於單次發作組,在憂鬱症復發組當中NLR、WBC皆顯著高於單次發作組,且有服用抗憂鬱藥物的患者中NEUT、CRP、NLR數值顯著高於無用抗憂鬱症藥物的人,不管是單次憂鬱症及憂鬱症復發之病患使用單一種藥物的患者中NEUT及CRP、WBC皆高於無使用藥物與使用兩種藥物的患者,在SSRI類藥物中,Cymbalta及SNRI類藥物中的Valdoxan此兩種藥物為憂鬱症復發病患首選藥物,本篇研究發現在排除條件之後PLR發炎因子與本篇研究無顯著之相關。
