Abstract
塵蟎過敏原(House dust mites, HDMs),如屋塵蟎Dermatophagoides pteronyssinus(D. pteronyssinus),是引發全球大多數過敏性氣喘與鼻炎患者致敏的主要因子。雖然目前藥物治療可緩解症狀,但部分患者仍存在長期耐受性有限及療效不佳的問題。過敏原免疫療法(Allergen-immunotherapy, AIT)為一種可改變疾病進程且能提供過敏症狀根本治療的方式,為患者提供了另一種選擇;然而,傳統的過敏原免疫療法(Conventional AIT)多以過敏原粗萃取物製成,存在安全性與療效上的疑慮。近年來,新型的過敏原免疫療法(Advanced AIT)已能標準化合成重組低致敏性蛋白質、特定表位的短鏈蛋白或合成過敏原核酸疫苗,以保留足夠的免疫原性並減少副作用發生。為克服現有治療的侷限,本研究透過整合IEDB中實驗驗證的B細胞表位資訊與預測軟體的結果開發一套評分系統,篩選具免疫優勢的區段,避免使用全長過敏原序列進行疫苗設計。基於此策略,我們設計了短序列多表位疫苗,包括Der p 1/2 mRNA-LNP與Der p 2-PreS融合蛋白,藉此排除非相關表位區域,並降低非預期免疫反應的風險。動物實驗結果顯示,兩種疫苗均能顯著提升致敏小鼠體內的Der p特異性 IgG1與IgG2a的抗體濃度,並於再次給予過敏原刺激後抑制IgE表現。此外,實驗結果也顯示多表位mRNA疫苗誘導偏向TH1的免疫反應,而Der p 2-PreS則顯示TH2傾向的免疫特性。本研究結果顯示,使用IEDB資料庫合併預測軟體進行表位篩選為一可行的疫苗設計方式且短序列多表位過敏疫苗具有作為塵螨過敏原免疫治療之潛力。
