Abstract
慢性阻塞性肺病 (COPD)是一種慢性發炎的疾病,其特徵是增強且持續的發炎反應。許多證據支持第1型和第17型T輔助細胞驅動的自體免疫反應可能是關鍵的機制。自體抗原的產生和免疫耐受性的降低進而觸發持續的自體反應性的T或B細胞反應,對於啟動和發展自體免疫至關重要。在慢性阻塞性肺病中,新生自體抗原的來源可能來自嗜中性球和巨噬細胞所釋放的蛋白水解酶對肺實質細胞外的基質,特別是彈性蛋白纖維的降解所形成。除了新生的自體抗原產生外,另一個導致自體免疫反應的重要機轉-免疫耐受性下降的機制則研究較少。程序性死亡配體1(PD-L1)是一種在抗原呈現細胞 (APC) 上表現的共同抑制分子。 PD-L1在APCs上的表現是調控免疫耐受性的機制。因此,我們研究代表耐受性的表現PD-L1的樹突狀細胞 (DC) 及其亞型,以及它們與特異性T細胞免疫和COPD臨床表型的關係。我們使用流式細胞儀分析病人及健康者周邊血單核細胞(PBMC)中表現PD-L1的樹突細胞(DC)及其亞型。以流式細胞儀分析在彈性蛋白作為自身抗原刺激下的T細胞的增殖反應,並使用酵素連結免疫吸附測定 (ELISA) 測量細胞激素。本研究共有 83 位參與者(慢性阻塞性肺病,n = 54;正常,n = 29)。 研究顯示COPD 患者 PBMC 中的 PD-L1+第1型傳統樹突狀細胞 1 (cDC1) 顯著減少(13.7±13.7%,P = 0.03)。 PD-L1+ cDC1 的減少與 COPD的肺功能快速下降的表現型相關(P = 0.02),其下降與T細胞免疫中的CD4+T 細胞有關(r=-0.33,P=0.02)。從功能上講,如果我們在健康者的PBMC中給予PD-L1阻斷劑並在CD3/彈性蛋白刺激下,CD4+T細胞會顯著增生(31.2±22.3%,P=0.04)。同樣的給予PD-L1阻斷劑並在CD3(156.3±54.7,P=0.03)和CD3/彈性蛋白刺激下的白細胞介素(IL)-17A的產生亦會顯著增加(148±64.9,P=0.03)。 然而,若先將cDC1移除,PD-L1阻斷劑即未能導致IL-17A 的產生。相較之下,加入PD-L1阻斷劑後的干擾素 (IFN)-γ、IL-4 和 IL-10 沒有顯著變化。因此,我們研究的結論發現血液中耐受性PD-L1+cDC1在COPD中顯著減少,特別是在肺功能快速下降的表現型。在機轉部分,耐受性PD-L1+cDC的減少會導致對自身抗原的耐受性降低進而導致CD4+ T 細胞增生及趨向Th17為主的持續發炎。恢復免疫耐受性可能有助於COPD的治療,而耐受性PD-L1+cDC1可能為COPD提供一種新的治療策略。
