Abstract
背景: 痛風是男人和停經後的女人最常見的一種關節炎,臨床表現為劇烈的關節疼痛,伴隨關節及周圍軟組織出現紅腫熱痛。痛風造上經濟上的負擔是無法忽視的,尤其是以痛風發作次數頻繁的病人,其經濟負擔更是加劇。儘管台灣是痛風普及率高的國家之一,但是我們對於痛風或痛風發作頻繁病人所造成的經濟負擔是不清楚的。因此,這篇研究的目的就是要了解痛風和非痛風以及痛風發作頻繁和不頻繁之間醫療資源利用與花費的差別。以及在急性痛風發作時候,醫生開立處方的情形。

方法: 本文是利用全民健康保險資料庫來做的一個回溯性性研究。根據預先定義的痛風病人和的痛風發作的標準,我們找出了痛風病人和其一年內發作次數。將痛風病人以1:3配對的方式找到非痛風病人。再將痛風發作次數至少三次以上的病人,以1:1配對的方式找到痛風發作次數小於三次的病人。去看痛風和非痛風以及痛風發作頻繁和不頻繁的病人,一年內所有原因或痛風相關醫療資源的利用與花費。另外,也進一步去分析急性痛風發作時候的用藥。

結果: 在2010年的百萬歸人檔中,2011年有993,332受保人。在其所有受保人中,有21,376為配對到的痛風病人。和非痛風病人相比,痛風病人有較多的門診看診次數(平均數: 31.2 vs. 22.6)、住院次數(平均數: 1.7 vs. 1.5)、以及急診看診次數(平均數: 1.9 vs. 1.7)(所有 p < 0.0001)。在花費方面,痛風病人比起非痛風病人有較多的門診花費(中位數: 美金 $634 vs. $404)、住院花費(中位數: 美金 $1,330 vs. $1,254)、以及急診花費(中位數: 美金 $87 vs. $89)(所有 p < 0.05)。

在21,376痛風病人當中,有3,553為為配對到且痛風發作次數至少三次以上的病人。平均所有醫療資源利用在痛風發作頻繁的病人比起痛風發作不頻繁的病人較為高(35.9 vs. 30.7; P < 0.0001)。然而,在醫療資源花費的中位數,兩組是差不多的(p = 0.25)。而痛風相關醫療資源利用(22.7 vs. 15.6; p < 0.0001)與花費(美金 $369 vs. $287; p < 0.0001)在痛風發作頻繁的病人都比起痛風發作不頻繁的病人較為高。另外,超過55.8%的急性痛風發作以colchicine + NSAIDs的用藥為主。

結論: 痛風病人在醫療資源利用與花費都比非痛風病人來得較高。在痛風病人當中,醫療資源利用是以門診為主,而醫療資源花費是以住院為主。在痛風發作頻繁的病人中,其醫療資源利用與痛風相關花費都比起痛風發作不頻繁的病人為高。
