Abstract
研究背景
Regorafenib為一種口服多標靶小分子抑制劑,可用於接受過第一、二線療法的轉移性大腸直腸癌患者。在一項樞紐性第二期試驗中,regorafenib的劑量調整策略不僅可達到與標準劑量相近的療效,還能減少副作用的產生。不過,有文獻指出,在真實世界中低起始劑量的regorafenib通常與低累積劑量有關,而後者會使得整體存活期變差。考慮到上述的待解問題,以及尚未有更新的本土證據,本研究旨在比較標準起始劑量與低起始劑量的regorafenib之療效與安全性。

研究方法
本研究屬於回溯性觀察性研究,使用多中心醫院資料庫——台北醫學大學臨床研究數據庫進行分析。本研究納入族群為二十歲以上、於2014年1月1日至2021年12月31日使用regorafenib治療轉移性大腸直腸癌的病患。研究族群依照起始劑量分為標準起始劑量組(160毫克)與低起始劑量組(<160毫克)。主要研究指標為治療終止時間 (time-to-treatment discontinuation),次要指標則為整體存活期 (overall survival) 與五項regorafenib常見副作用的發生率,以敘述性統計與Kaplan-Meier方法來比較兩種起始劑量的療效與安全性;預後因子則透過Cox比例風險模型來進行分析。除此之外,本研究也規劃了兩個次族群分析與三個敏感性分析,用以測試研究結果的穩健性。

研究結果
本研究共計納入266位病人,其中58位為標準起始劑量組、208位為低起始劑量組。標準起始劑量組與低起始劑量組的治療終止時間中位數分別為68.0天與64.5天 (p = 0.25);而標準起始劑量組的整體存活期中位數顯著優於低起始劑量組(9.7個月vs. 6.7個月,p = 0.01)。在多變量Cox迴歸分析中,低起始劑量與存活呈現負相關(風險比1.66 [95% 信賴區間1.22–2.30])。另一方面,低起始劑量組的手足皮膚反應 (hand-foot skin reaction) 與總膽紅素上升的發生率較低(p = 0.01與0.03)。在敏感性分析中,經排除併用其他抗癌藥物的病人後,兩研究劑量組的整體存活期中位數轉為沒有顯著差異;除此之外,排除先前曾使用過TAS-102的病人會導致兩組的整體存活期減少,但標準起始劑量組仍顯著優於低起始劑量組。

結論
本研究顯示標準起始劑量組與低起始劑量組的治療終止時間無顯著差異。值得注意的是,低起始劑量組的整體存活期較短,但該組病人發生手足皮膚反應與總紅素上升的副作用機率較低。對於可承受標準劑量regorafenib的病人而言,併用其他抗癌藥物可能具有潛在療效效益,不過其可行性與安全性方面仍需更多證據支持。
