Abstract
大腸直腸癌(colorectal cancer, CRC)為全球第三常見的癌症,死亡率亦居高不下,台灣更長年位居大腸癌發生率全球前列。臨床治療上,5-Fluorouracil(5-FU)為最常使用的第一線化療藥物,惟患者常因產生原發性或繼發性抗藥性,導致治療反應下降,嚴重影響預後與存活率。因此,發展可克服5-FU抗藥性的治療策略,已成為當前大腸癌治療的重要課題。乳癌則為全球女性罹癌率與死亡率最高的癌別,其根據分子標誌可分為荷爾蒙受體陽性(HR⁺)、人類表皮生長因子第二型受體陽性(HER2⁺)及三陰性乳癌(Triple-Negative Breast Cancer, TNBC)等亞型。TNBC由於缺乏可供治療的荷爾蒙與HER2受體,無法使用標靶或內分泌療法,且復發率與轉移風險高,為臨床治療最具挑戰的乳癌亞型。相較之下,HR⁺型乳癌可使用內分泌治療或CDK4/6抑制劑,但部分患者仍可能產生耐藥性與復發,亦需新型治療方式以延長疾病控制。隨著腫瘤免疫與發炎機制的研究進展,抗發炎藥物在癌症治療領域受到廣泛關注。慢性發炎被認為是腫瘤發展與抗藥性的重要推手,許多癌症中常見的發炎訊號如NF-κB、COX-2、IL-6等均與腫瘤增生、轉移與治療反應不佳有關。因此,抗發炎藥物除可調控發炎反應,亦具抑制腫瘤生長與增強化療敏感性的潛力。現有研究已顯示,無論是合成藥物如celecoxib、dexamethasone,或天然化合物如薑黃素、白藜蘆醇、丹參酮(Tanshinone)類化合物等,皆可能透過抑制發炎路徑、誘導凋亡或干擾腫瘤代謝,增進抗癌療效並降低副作用。本研究篩選十種具有抗發炎活性的天然與合成化合物,評估其對大腸癌與乳癌細胞株的抑制作用。實驗設計上,首先以MTT分析細胞活性變化,評估藥物毒性;再以wound healing及colony formation測試癌細胞遷移與群落形成能力;並透過Western blot分析凋亡、自噬及鐵死亡等相關蛋白質表現,探討可能的細胞死亡機轉。同時為評估藥物安全性,使用斑馬魚進行體內毒性測試,並於小鼠異種移植模式驗證候選藥物對大腸癌的抗腫瘤效果。在大腸癌部分,針對一般細胞株與具5-FU抗藥性的細胞株進行比較,結果顯示Timosaponin Alll與Tanshinone IIA對抗藥性細胞具顯著抑制效果,具有克服抗藥性的潛力。乳癌實驗中選用MCF7(HR⁺)與MDA-MB-231(TNBC)細胞,發現Timosaponin Alll與蛋白酶體抑制劑Delanzomib對兩者皆具細胞抑制能力,尤以TNBC細胞表現最顯著,推測可能透過抗發炎與蛋白質降解路徑協同發揮作用。綜上所述,本研究顯示具有抗發炎特性的藥物可望克服化療抗藥性,並對如TNBC等難治型癌症展現治療潛力,未來可進一步深入其分子機轉,作為發展新型輔助療法的基礎。
