Abstract
研究背景:
類風濕性關節炎(Rheumatoid Arthritis, RA)是一種常見的慢性自體免疫性疾病,病人除了關節發炎及功能受損外,還面臨多種系統性併發症的風險,尤其是重大心血管事件(Major Adverse Cardiovascular Events , MACE),其發生率及合併死亡率顯著高於一般人群。然而,目前針對類風濕性關節炎病人併發重大心血管事件的研究多基於傳統統計模型,未能充分考量時間依賴變數與動態特徵。鑑此,本研究旨在建立可隨時間變化預測類風濕性關節炎病人重大心血管事件風險的機器學習模型,並能提供個案管理師制定更有效的病人預後管理計劃。
研究方法:
本研究篩選了2011~2022年臺北醫學大學臨床資料庫類風濕性關節炎的門診病人數據,包括病人基本資料、疾病史、臨床檢驗結果和用藥紀錄,最終納入了2,461名類風濕性關節炎病人,其中253名(10.3%)在研究期間發生重大心血管事件。使用Cox-PH比例風險模型、隨機生存森林模型(RSF)、Cox-time及DeepSurv深度學習模型以預測時間事件風險模型,透過C-Index和IBS指標分別評估事件風險排序的準確性與預測誤差。此外,為提升臨床應用價值,本研究結合特徵置換重要性(Permutation Importance)與SHAP方法,進行模型可解釋性分析。
研究結果:
單變量與多變量Cox迴歸分析顯示,診斷年齡、糖尿病、高血壓、高血脂、慢性阻塞性肺病、間質性肺炎、免疫調節抗風濕藥物、膽固醇藥物、類風濕因子、Creatinine、ESR與病人的預後密切相關。基於隨機生存森林模型的風險預測結果表現最好(C-index=0.8771、IBS=0.0775),使用Permutation Importance與SHAP方法分析模型結果顯示,Creatinine、GPT、CRP、免疫調節抗風濕藥物及診斷年齡等特徵均被視為風險重要特徵;SHAP結果表示診斷年齡、慢性阻塞性肺病、高血脂、糖尿病與糖尿病藥物服用天數為MACE的風險因子;免疫調節藥物、生物製劑、類固醇及膽固醇服用天數則是保護因子。
結論:
本研究證明基於機器學習隨機生存森林模型在類風濕性關節炎病人併發重大心血管事件風險預測的可行性,結合Permutation Importance與SHAP方法則為臨床應用提供高風險因子及可解釋的預測,更可能有助於個案管理師改善病人預後。
