以醫療經濟考量Procalcitonin (PCT)與CRP對臨床診斷敗血症的影響

Abstract
本實驗室發現有時PCT的結果與病人臨床上症狀並不符合,為了檢討PCT在敗血症上的評估如何達到最大效益,我們收集了九十九年二月至七月的病患檢驗結果,做細菌培養的有3933筆、PCT 303筆、CRP 5668筆的病患檢驗報告,以SAS軟體統計,並加以分析檢討。其中有做細菌培養及PCT的共有196筆;有做細菌培養及CRP的共有2366筆,數據顯示PCT、CRP對於細菌培養陽性結果的AUC(Area under the ROC curve )分別為0.828(p value=0.0001)及0.802(p value=0.0001)相對於CRP而言,對於細菌培養陽性結果PCT的預測率較好。當PCT cut-off 值定在0.882時敏感性可達最佳狀態(72%);特異性(80.12%)。所以,我們進一步發現了當PCT濃度介於0.05~0.882 ng/mL時,應加做CRP,便能提高敏感性至100%(cut-off 值:5.53 mg/dL);當PCT濃度介於0.05~2.00 ng/mL時,加做CRP亦能提高敏感性至100%。先前研究建議,當PCT濃度介於0.5和2.0 ng/mL時,最好能在6~24小時內重新測定PCT。然而,在現行健保制度下易被核刪且被放大罰緩,健保給付共需2000元,若能搭配CRP的結果,則只需一次PCT及一次CRP共需給付1250元,如此不但能提高敏感度,更能在最經濟的狀態下,早期診斷與治療敗血症,降低病人的死亡率,並解決目前濫用抗生素的問題,減少細菌抗藥性的產生。
