Abstract
患有糖尿病 (diabetes mellitus, DM) 的女性較易罹患婦科炎症,並伴隨慢性盆腔疼痛 (chronic pelvic pain, CPP) 內臟痛覺過敏與功能性腸胃道障礙 (functional gastrointestinal disorders, FGID) 等合併症,但其潛在機制尚未完全明確。先前研究指出,DM引起的自主神經病變 (diabetic autonomic neuropathy, DAN) 可能促進感覺神經活性增加,進而上調脊髓興奮性,導致神經病變性疼痛。此外,血清和糖皮質激素誘導激酶1 (serum- and glucocorticoid-inducible kinase 1, SGK1) 在神經損傷及發炎狀態下表現量升高,可能參與中樞神經的疼痛敏化。因此,本研究假設DM可能透過SGK1的活化,敏化子宮-結腸交互作用。本研究以鏈脲佐菌素 (streptozotocin, STZ) 誘導糖尿病大鼠,在第二週進行血糖測試確認模型建立成功,並於第四週進行結直腸撐張 (colorectal distension, CRD) 測試,同時使用芥子油 (mustard oil, MO) 對子宮進行急性刺激,來評估內臟運動反應 (visceromotor response, VMR)。結果顯示,NAV組與DM組的VMR反應隨MO濃度呈劑量依賴性增加,而DM組的VMR反應則顯著高於NAV組,證實DM狀態會敏化子宮-結腸交互作用。此外,在STZ誘導後第三週進行鞘內注射SGK1抑制劑 (GSK-650394) ,於第四週進行CRD測試。結果顯示DM+GSK組明顯抑制了VMR反應,證實SGK1抑制能夠降低DM大鼠的VMR反應,此結果進一步驗證SGK1在DM相關內臟敏化過程中具有關鍵調控作用。因此,SGK1可能為DM女性患者內臟疼痛之潛在治療標靶,對未來內臟疼痛的治療具有重要臨床應用價值。
