Abstract
侵襲性真菌感染的發病率隨著免疫系統受損之病患的增加而顯著升高,侵襲性真菌感染所引發的系統性感染症在臨床上有著高的發病率和死亡率。真菌的檢測主要是以真菌培養為標準方法,傳統鑑定主要是依據真菌的形態、生長以及生理生化等特徵對真菌進行分類。然而真菌的種類繁多,個體多態性明顯,而且其生長、生理生化特徵也會隨著環境的變化而不穩定。傳統真菌診斷的問題,需另外尋找準確且快速的鑑定方法。本研究主要以侵襲性真菌感染中真菌菌血症(Fungemia)為研究族群。以新北市某醫學中心菌庫中所收集的真菌菌株進行培養、DNA的萃取,選取ITS (internal transcribed spacer)基因序列將該片段進行PCR增幅,接著以Sanger定序法進行DNA定序,藉由DNA序列差異鑑別真菌菌種,ITS區域中的變異區段具有菌屬或菌種特異性,以及菌株間的鹼基對差異性,可廣泛偵測到多數真菌種類。接著利用剩餘的全血血品作基質,模仿真菌菌血症檢體進行檢測,建立以PCR-DNA定序為基礎之分子生物學方法於臨床檢體鑑定真菌。本研究一共測試真菌分類共七菌屬十六菌種,包括Candida species (6種)、Aspergillus species (4種)、Cryptococcus neoformans、Fusarium species、Penicillium marneffei、Trichosporon asahii、Rhodotorula species,及Fusarium species。所有菌株(含參考菌株9株及臨床分離菌株154株)以ITS序列分析鑑定之一致性為95.09 %(155/163),全數可鑑定至菌屬程度以上。另外,Panfungal PCR分析靈敏度為100 fg/uL Candida albicans genomic DNA及模擬真菌菌血症血液105~560 CFU/mL。ITS定序分析是一種深具潛力的鑑定工具,可提供臨床真菌診斷輔助。
