Abstract
暫態受器電位香草第一類受器 (transient receptor potential vanilloid type 1;TRPV1受器) 是非選擇性陽離子通道,主要可通透鈣及鈉離子。大多分佈於肺及皮膚等C纖維感覺神經,並負責多種訊息的接收 (polymodel sensation)。肺C纖維感覺神經是種「類痛覺」的感覺神經,刺激肺C纖維感覺神經可引發多種呼吸反射,例如:肺化學反射 (呼吸停止、心跳減慢、血壓降低)、咳嗽、氣道收縮及黏液的分泌。文獻指出,在組織發炎時,TRPV1受器在皮膚C纖維感覺神經的敏感化,及其所導致的疼痛反應增強當中,扮演極為重要的角色。然而,活化肺中這些受器,是否也能使肺C纖維感覺神經更敏感而引發更強的呼吸反射,仍不知道。因此,本研究擬以內生性配體 (endogenous ligand) 刺激TRPV1受器,探討其對於刺激肺C纖維感覺神經引發的肺化學反射敏感度的影響及其機轉,TRPV1受器內生性配體種類繁多,本實驗將選擇相對穩定以及強度較強的N-Arachidonoyldopamine (NADA)當作TRPV1受器的活化劑。將觀察動物呼吸型態及心血管反應去探討 (1) NADA是否能夠增強刺激肺C纖維感覺神經所引發的心肺反射 (2) NADA增強的心肺反射中,TRPV1受器參與的重要性。以麻醉、自發性呼吸的大白鼠為動物模式,共紀錄30隻動物。自頸靜脈注射腺苷酸 (adenosine, 0.2-0.6 mg/kg) 進入肺循環,引發些微的呼吸反射及心血管反應。在靜脈灌流NADA (200 μg/kg/min, 0.5 ml/min for 2 min) 溶液後,給予一樣劑量的腺苷酸,產生相較於未給予NADA前,增強18倍的呼吸暫停反應,及伴隨更強烈的心血管反應。NADA所增強的反應在截斷迷走神經及阻斷C纖維感覺神經傳導後消失,得知NADA所增強的反應是藉由C纖維感覺神經傳導而來。NADA所增強的反應在給予TRPV1受器拮抗劑capsazepine後消失。根據以上結果,得到結論是NADA可增強腺苷酸所引發的心肺反射,而此增強效果可能是藉由活化肺C纖維感覺神經末梢的TRPV1受器導致。
