Abstract
背景:結核病是造成全球最多死亡的單一感染源之一。現今結核病治療的副作用、長週期,以及多重抗藥性菌株的問題,使得治療效果不盡理想,因此,新型態抗結核藥物的發展刻不容緩。Glutamine synthetase為結核菌生長過程中不可或缺的酵素,抑制此酵素可阻礙結核菌的生長、降低感染力,為潛在的抗結核病機轉。雙磷酸鹽類為治療骨質疏鬆症的藥品,被發現具有glutamine synthetase抑制效果,然而,目前尚未有研究探討雙磷酸鹽類對於人類結核病的效果。本研究利用台灣健保資料庫,探討雙磷酸鹽對於結核病的預防效果。

材料與方法:本回溯式世代研究利用台灣全民健保資料庫與疾病管制署結核病資料庫,以2007至2014年間被診斷為骨質疏鬆症,且於診斷後一年內有骨鬆相關骨折的病人作為研究族群,依照雙磷酸鹽類藥物的使用情形,進一步分為用藥組與非用藥組。以用藥組為基準,利用傾向分數1:1配對非用藥組,用Kaplan-Meier method分析兩年內發生結核病的情形,並以Cox proportional hazard model探討發生結核病的相關危險因子。統計軟體使用SAS 9.4版,統計顯著性設定為absolute standardized mean difference > 0.1或p < 0.05。

結果:用藥組與非用藥組分別有46,842人和119,282人符合納入條件。以用藥組作為基準,利用傾向分數1:1配對性別、年齡、共病症與併用藥物,兩組各有46,842人進行後續結果分析。兩年觀察期內共有723人發生結核病,其中348人為用藥組,375人為非用藥組,兩組發生人數無顯著差異(p = 0.313);追蹤天數方面,用藥組平均追蹤時間比非用藥組長(696.3 ± 123.2天vs. 688.9 ± 136.2天, p < 0.001)。Kaplan-Meier存活分析顯示,有無使用雙磷酸鹽類並不影響結核病的發生機率(p = 0.252, log-rank test),此結果與multivariate Cox proportional hazard regression結果一致(adjusted hazard ratio: 0.86, 95% confidence interval: 0.71-1.04, p = 0.125)。此外,高齡、男性、臥床狀態和使用類固醇者,發生結核病的風險較高;反之,使用抗血小板藥物或statin類藥物者,發生結核病的風險較低。

結論:臨床資料顯示,使用雙磷酸鹽類藥物無法降低骨質疏鬆症病人發生結核病的機率;高齡、男性、臥床和類固醇使用者為結核病的高危險群,若上述族群出現疑似結核病症狀,須提高警覺,避免延誤治療。
