Abstract
甲基乙二醛(Methylglyoxal, MG),來自於人體各種能量代謝轉換時的旁生產物,其高反應性除了產生糖化終產物外還會造成組織的損傷,而可以把MG代謝為D-乳酸的glyoxalase pathway則被視為解毒系統。D-乳酸在人體中含量極低而且缺乏有效率的代謝酵素,因此當人體發生損傷時,則可以在尿中檢測到D-乳酸上升。本研究將探討以MG、D-乳酸的特性是否能夠用於評估免疫性腎炎的疾病變化,故導入加速型腎毒血清(NTS)腎炎的小鼠模型,探討在免疫病態模型之中,MG、D-乳酸在尿中的變化。
實驗動物為C57BL/6J雌性小鼠,分為normal組與NTS組,NTS組在施打NTS前5天預先皮下注射白兔免疫球蛋白(IgG)。收集NTS注射前9天(day -9)、前2天(day -2)、後5天(day 5)、後12天(day 12)的尿液分析其尿蛋白、MG、D-乳酸,並於注射NTS後15天犧牲採血及腎臟進行SCr、BUN等生化檢驗以及組織切片鏡檢。
結果發現,NTS誘導的小鼠腎臟於組織鏡檢可觀查到血栓、新月體等典型特徵,而且尿蛋白在第5、12天顯著高於正常小鼠(day 5: 36.2 ± 5.47 vs. 13.0 ± 2.25 mg/day, day 12: 31.7 ± 5.18 vs. 10.6 ± 1.02 mg/day),MG與D-乳酸在第5、12天也顯著高於正常小鼠 (MG/UCr day 5 Normal: 85.5 ± 14.7, NTS: 29.1 ± 5.26 μg/mg; day 12 Normal: 6.85 ± 2.28, NTS: 21.8 ± 6.16 μg/mg) (D-lactate/UCr day 5 Normal: 0.18 ± 0.0059, NTS: 0.34 ± 0.021 μmol/mg; day 12 Normal: 0.14 ± 0.0055, NTS 0.31 ± 0.038 μmol/mg)。
本實驗證實在加速型腎毒血清腎炎的病態中,MG與D-乳酸相比於尿蛋白,在正常、損傷組都有相同的趨勢,可以反應在免疫性腎炎病態中腎臟的損傷情況。具有後續研究價值,以開發更簡便的腎損傷檢驗指標。
