Abstract
背景:心血管疾病(CVDs)是全球主要的死亡原因,其危險因子包括高血壓、糖尿病、肥胖、吸菸與飲酒。Framingham 風險評分(FRS)常用於評估心血管疾病風險。近期有研究指出,CVDs 與表觀遺傳學,特別是DNA甲基化,有所關聯,因其可能影響基因表現。然而,目前在台灣針對DNA甲基化與心血管疾病之間關係的研究並不多。本研究旨在探討全基因體DNA甲基化與心血管代謝異常和心血管疾病風險之間的相關性,並進一步分析其是否為生活習慣因子與 CVDs 風險的中介因子。
方法:橫斷式研究共納入2,439位來自台灣人體生物資料庫的參與者,其中有526位受試者進一步納入世代研究。研究中使用邏輯斯迴歸來探討全基因體DNA甲基化(以LINE-1與Alu代表)與心血管疾病(CVDs)、心血管代謝異常(CMD)、以及10年心血管疾病風險(以Framingham風險評分代表)之間的關聯性。同時,使用Cox比例風險迴歸以釐清生活習慣因子(包含吸菸、飲酒、肥胖及規律運動)與心血管疾病之間的關係。使用R軟體中的Mediation套件,分析全基因體DNA甲基化對生活習慣因子與 CVDs 風險關聯性的中介效應。
結果:在控制干擾因子後,發現Alu高甲基化組有較高的異常LDL-C風險(勝算比(OR) = 1.234,95%信賴區間(CI):1.047–1.455,P-value = 0.0124)。此外,LINE-1與Alu高甲基化組,其 FRS 超過10%的風險也較高,LINE-1的OR為1.395(95% CI: 1.107–1.759,P-value = 0.0049),Alu的OR為1.261(95% CI: 1.002–1.588,P-value = 0.0485)。中介效應分析結果顯示,全基因體DNA甲基化並未調控生活習慣與 CVDs 風險之間的關係。
結論:全基因體DNA甲基化與異常的LDL-C及未來 10 年內心血管疾病風險升高有關。然而,全基因體DNA甲基化在生活型態與心血管疾病風險之間不具中介效應。
