Abstract
大腸直腸癌是第三常見的癌症,並且是所有癌症中造成死亡的第四名,而癌症轉移是造成大部分癌症病人死亡的主要原因之一,若能夠對於癌症細胞轉移機制有更深入的瞭解,將能夠有助於發展預防或治療癌症轉移的藥物,過去的流行病學研究中指出使用非類固醇類抗發炎藥物(Non-steroidal anti-inflammatory drugs,NSAIDs)能夠降低大腸直腸癌的發生率及改善預後,然而此保護效果背後的分子機轉至今仍未被釐清。
鈣池調控鈣離子通道(Store-operaed calcium channel,SOC)對於非興奮型細胞(如:癌細胞、免疫細胞)的胞內鈣離子訊息傳遞扮演了重要角色,有關鈣池調控鈣離子內流(Store-operaed calcium entry,SOCE)與癌細胞遷移能力的關係在乳癌細胞上首次被發現,將鈣離子感測分子STIM1做基因靜默造成乳癌細胞的遷移及侵入能力被抑制,後續的研究也顯示抑制SOCE會影響包括子宮頸癌、肝癌、膠質母細胞瘤、黑色素瘤、大腸直腸癌等癌細胞的遷移及侵襲能力,其中,研究中發現在低COX-2表現的大腸直腸癌細胞上,使用ibuprofen及indomethacin仍然能夠廢除大量表現STIM1所增強的遷移能力,因此,本篇研究假設有某些NSAIDs可能會經由影響SOCE進而影響大腸直腸癌細胞的遷移。
為了證明此項假設,我們使用大腸直腸癌細胞株SW-480、HCT-116作為實驗平台,測試了包括aspirin、ibuprofen、indomethacin、ketoprofen、diclofenac、piroxicam、sulindac、rofecoxib等8種不同的NSAIDs對於細胞增生、遷移及SOCE的影響,結果發現,我們所測試的8個NSAIDs能夠顯著的抑制兩株細胞株的細胞增生,並且都能夠抑制STIM1表現量高的SW-480細胞株的細胞遷移,然而只有ibuprofen及indomethacin能夠抑制STIM1表現量低的HCT-116細胞株的細胞遷移,接著我們測試NSAIDs對於SW-480細胞株上SOCE的影響,在8個NSAIDs中,我們發現aspirin、sulindac及低濃度的indomethacin有抑制SOCE的效果。
根據我們的實驗結果及過去的文獻,aspirin、sulindac及indomethacin有可能藉由抑制大腸直腸癌細胞的鈣離子訊息傳遞路徑而抑制細胞遷移能力,除此之外,STIM1有潛力做為生物標記來預測適合使用NSAIDs治療,在未來研究中應更進一步於動物實驗平台、臨床資料庫驗證此研究結果。
