Abstract
糖尿病腎病(Diabetic nephropathy、DN、糖尿病腎病變) 是由腎小球中毛細血管的損傷引起的漸進性腎臟疾病,透析是許多開發國家治療的主要指標。如果糖尿病腎病持續的惡化,慢性腎功能衰竭的併發症可能會發生較早,而且會更迅速的進展,即使透析或移植已經開始治療,糖尿病患者的病情也會比非糖尿病患者情況來的差。對於臨床糖尿病腎病變而言,它是一個漸進性的,約於糖尿病診斷後15到25年發病,影響了25到30% 的30歲以下患者。這也是年輕糖尿病患者早死的主要原因。糖尿病腎病是慢性腎臟疾病和終末期腎病的常見原因,如果血糖沒有好好的控制,風險就會更高。所以糖尿病腎病的檢測目前在研究中是非常重要的課題。
本次實驗檢體使用健康控制組26個,糖尿病患者70個以及糖尿病腎病患者70個。所有檢體經過刀豆A親素和層析法(Con A),利用醣蛋白來發現糖尿病腎病展程中的差異蛋白。我們挑選六對檢體經奈米級液相層析串聯質譜,鑑定出表現量有差異的醣蛋白。鎖定的研究主軸為脂蛋白酶元A-IV(也稱為APOA4)。挑選14對檢體以西方墨點法定量該蛋白質的表現量,再以Con A免疫酵素連結反應檢測健康控制組26個,糖尿病患者70個以及糖尿病腎病患者70個。
APOA4血清醣蛋白在西方墨點法的表現量,糖尿病患者是健康控制組的1.69倍(P=0.0243),曲線下面積 0.699、靈敏度 85.71% (95%CI=57.2% ~98.2%)、特異性 57.14% (95%CI=28.9% ~82.3%)。糖尿病腎病患者是健康控制組的3.28倍(P<0.0001),曲線下面積 0.964、靈敏度 92.86% (95%CI=66.1% ~99.8%)、特異性 92.86% (95%CI=66.1% ~99.8%)。
APOA4血清醣蛋白在免疫酵素連結反應的表現量,糖尿病患者是健康控制組的1.91倍(P<0.0001),曲線下面積 0.817、靈敏度 68.57% (95%CI=56.4% ~79.1%)、特異性 65.38% (95%CI=44.3% ~82.8%)。糖尿病腎病患者是健康控制組的2.56倍(P=0.0001),曲線下面積 0.880、靈敏度81.43% (95%CI=70.3% ~89.7%)、特異性 73.08% (95%CI=52.2% ~88.4%)。
本次研究我們是以蛋白質體學的方式進行探討,使用刀豆素A吸附血清醣蛋白親合層析法,進入奈米級液相層析串聯質譜中分析、量化,尋找血清中新穎的生物標記。以上數據表明,不管是在西方墨點法或是免疫酵素連結反應,APOA4醣蛋白在不同階段皆有顯著性的差異。
