Abstract
隨著近年人工智慧技術興起,醫療領域亦邁向智慧化轉型,特別是大型語言模型近年在醫療數據處理與臨床決策支持的應用上發展迅速。而急診室作為醫院中全天運作、收治各類急重症病患的單位,長期承受高度負荷與壅塞壓力。急診壅塞已被證實與病患治療延誤、住院與死亡風險上升、醫療費用增加及病患滿意度下降密切相關。過去研究顯示,急診室壅塞期間病患死亡率顯著提升,抗生素治療延誤、 ICU 收治延宕及整體醫療服務品質下降等問題亦更為普遍。又中風作為全球主要死因與致殘原因之一,其風險的快速識別尤為重要。2017 年中風在非傳染性疾病中死亡率排名第二,占全球總死亡人數 11%;全球 25 歲起終生中風風險約 24.9%,東亞(含台灣)更高達 38.8%。台灣在 2019 年的中風相關死亡人數近 12,000 人,創近 12 年新高。中風病患若能在急診階段及早辨識並獲得及時處置,有助於減少後續神經功能損傷並改善預後。傳統中風風險預測工具多仰賴繁瑣的人工判讀或高度結構化資料,不易於急診環境中快速落實。面對嚴峻挑戰,有效的智慧化輔助系統如何在急診第一線快速完成中風風險評估並即時介入,是改善病患預後與醫療效率的關鍵。
本研究融合結構與非結構化資料建構多模態模型,支援臨床快速決策。並利用大型語言模型與機器翻譯技術,取代傳統自然語言處理、語言統一、簡寫解析等耗時作業,以簡化流程、提升資料標準化與模型解讀能力。此外,本研究針對中風資料集中常見的類別不均衡問題,探索採樣技術及大語言模型生成數據的應用效益。研究同時比較原始文本、翻譯文本及語言模型改寫文本的預測表現,評估不同處理策略對模型準確性與穩健性的影響。預期成果將有助於促進急診中風風險預測之即時性、精準性與臨床適用性,並為優化急診分流、緩解壅塞問題及強化公共衛生應對能力提供具體貢獻。
