Abstract
中文摘要

研究背景:
多重用藥治療的議題,在抗乙醯膽鹼藥物負荷的相關安全性風險也越來越受到關注,尤其是會使用特定某些種類乙醯膽鹼藥物治療的病人族群,如巴金森氏症病人。巴金森氏症病人會用到的抗乙醯膽鹼藥物外,也會因其他共病及症狀會再使用到其他具有抗乙醯膽鹼作用之藥物,因此可預期巴金森氏病人其暴露在抗乙醯膽鹼藥物之副作用與不良反應的風險也比一般人高。

研究目的:
本研究期透過系統性回顧與統合分析了解抗乙醯膽鹼藥物負荷對於巴金森氏症病人的影響。

研究方法:
研究搜尋Pubmed、Embase、Cochrane Library和Web of Science資料庫,萃取相關原始數據,並以RevMan (5.3 version ; Cochrane Collaboration,Oxford,UK)進行統合分析,使用Newcastle-Ottawa Quality Assessment Scale評估所收入的文獻品質 。

研究結果:
本研究共納入8篇研究,皆為觀察性研究之文獻。共計人數為75853人,觀察組別(較高抗乙醯膽鹼暴露組)有53971人。平均年齡為73.5歲,女性比為46.7%,平均追蹤時間為18個月至8年。在主要觀察結果中,失智症風險 (OR:1.71, 95%CI: 0.93-3.17)以及認知功能(SMD: -0.71, 95%CI: -1.51,0.08),兩組在統合結果無顯著差異。進一步再以研究類型、抗乙醯膽鹼負荷評分量表種類、評分分數切點、以及研究地區進行次群組分析發現,研究類型與地區的次分析,降低了文章間的異質性(I2由87%降至14%)。其中地區的次分析中歐洲組別在兩組中有顯著差異,OR為2.91(95% CI: 1.02,8.28),但異質性高(I2:84%)。在次要觀察結果中,抗乙醯膽鹼負荷增加對巴金森氏症病人的骨折風險(OR:1.53, 95% CI: 1.28-1.84)與急診就診率(OR:1.25, 95%CI: 1.15-1.36),有顯著差異,且統合結果異質性低(I2:0%)。

研究結論:
巴金森氏症病人使用抗乙醯膽鹼藥物而發生失智症的風險,種族是可能影響差異的因子之一。另外在其他不良反應結果如骨折風險以及急診就診率,是可能由抗乙醯膽鹼藥物引起,可考慮降階或替換其他藥物使用的可能性。
