Abstract
手術過程發生的低血壓,與病人發生不良的後遺症有關。整合相關因子的電腦輔助模型,可預測低血壓的危險性,對臨床麻醉會有不小的助益。此研究的目的即針對全身麻醉與脊髓麻醉的病患,發展類神經網路模型,以預測病患發生低血壓的危險性。
於第一部份研究,進行2007年3月至11的病歷回顧,共分析1017筆全身麻醉記錄。包含11個病患相關因子,2個手術相關因子,以及5個麻醉相關因子,用以建構類神經網路與邏輯迴歸模型。最後使用外部的資料進行模型的預測效能評估。3位臨床醫師針對同樣的病患資料,預測是否會發生低血壓。類神經網路模型,預測準確度為82.3%,敏感度為76.4%,特異度為85.6%。邏輯迴歸模型,預測準確度為76.5%,敏感度為74.5%,特異度為77.7%。類神經網路與邏輯迴歸模型,ROC曲線下面積分別為0.893與0.840。經過比較,臨床醫師預測的準確度與敏感度最低。
於第二部份研究,分析2004年9月至2006年12月的病歷,總共1501位病患接受半身麻醉的記錄,建構發展類神經網路與邏輯迴歸模型。75%的病患資料做用來訓練預測模型,於模型訓練完成後,利用其餘25%的資料進行模型的預測效能評估。5位麻醉醫師根據臨床經驗,針對相同的資料做出低血壓發生的預測。類神經網路模型預測的敏感度與特異度分別為75.9%與76.0%。邏輯迴歸模型預測的敏感度與特異度分別為68.1%與73.5%。ROC曲線下面積分別為0.893與0.840。類神經網路與邏輯迴歸模型,ROC曲線下面積分別為0.796與0.748。類神經網路模型的預測效能,明顯優於邏輯迴歸模型。臨床醫師有最低的敏感度,分別為28.7%,22.2%,21.3%,16.1%與36.1%。
針對全身麻醉的病人, 應用類神經模型來評估是否能實際改善低血壓的發生率。隨機選取34位病患為實驗組,先以類神經模型預測病患是否可能發生低血壓,並告知麻醉醫師,由醫師進行處置來預防低血壓的發生。隨機選取37位病患為對照組,先以類神經模型做出預測,但不告知麻醉醫師。最後計算低血壓發生率,實驗組與對照組分別為11.8%與29.7%,有統計上顯著的差異。應用類神經模型於臨床麻醉,實際降低了低血壓的發生率。
此研究所發展的類神經網路模型,針對全身麻醉與脊髓麻醉過程引起的低血壓,具有不錯的預測效能,亦擁有好的鑑別力與效正度。於臨床上使用,可提供臨床醫師決策支援的輔助,以及針對可能發生低血壓的高危險病人提高警覺性。
