Abstract
大腸直腸癌是全球普遍的癌症之一。根據2013年的統計,大腸直腸癌是台灣第三大主要癌症,死亡率高達 13%。大腸直腸癌發生的原因不完全清楚,目前已知可能與生活飲食習慣、環境因子有關,基因以及表觀基因(epigeneitc)變異也是導致大腸直腸癌的可能原因。在大腸直腸癌的臨床治療上,化學療法是手術治療後的常規治療,而化學療法的失敗,大部份是因為腫瘤抗性的產生。目前學界發現轉移性大腸癌患者對化學療法的反應率只有 20-30%,且單用 5-FU 的治療效果非常不佳,反應率僅10-15%;當 5-FU 與其他化療藥物(如 oxaliplatin、 irinotecan)並用時,則能將治療效果提升至較好的40-50%。為病人選擇正確有效的化療藥物,可說是大腸直腸癌治療上的關鍵。本研究認為若能發現特定生物標識,用以協助預測對特定大腸癌病患適合化學療法,進而提高病患的醫療品質,將具有重大意義。過去幾年,本研究持續尋找與大腸直腸癌預後相關的生物標記,並在兩百多個標記的篩選中,發現有15個標記在大腸直腸癌組織的表現量遠大於正常大腸組織。從臨床數據,本研究發現溶小體酸性磷酸酶 (lysosomal acid phosphatase 2,ACP2) 表現量高的病人,對5-FU的敏感性亦會增加。為進一步驗證前述現象,本研究將ACP2基因表象量高的大腸癌細胞株感染帶有shACP2的基因,使ACP2的表現降低,並觀察ACP2表現降低與細胞株對5-FU的敏感性的關聯性。研究結果顯示,ACP2表現量低的細胞株對5-FU的敏感性降低,並增加細胞增生以及抑制細胞凋亡的表現,同時增加細胞的移動(Migration)和侵入(Invasion)。綜上所述,本研究認為ACP2可作為預測大腸癌使用化學治療後的預後標誌,尤其是第二期大腸癌。
