Abstract
台灣地區女性癌症中,乳癌發生率為第一位,死亡率為第四位,發生高峰約在45-69歲之間,約為每十萬名婦女188-194人。西方國家的乳癌發生年齡在70至74歲,台灣的乳癌診斷年齡的中位數介在45~49歲之間,相較於西方國家的乳癌發生年齡,台灣乳癌病人發生年齡較年輕,因此探討臺灣乳癌致病機轉、是否有不同治療模式與指引、影響預後因子等的相關研究,顯得非常重要。過去的文獻分別指出SOX4、PLAU和PLAUR在肺癌、大腸直腸癌、乳癌和胰臟癌⋯⋯等癌症的表現量可作為癌症病人的預後、轉移或治療指標。因此,本研究探討SOX4, PLAU和PLAUR表現和乳癌病人預後的相關,尤其是ER、PR、HER2均陰性的三陰性乳癌,希望能找到其中的關聯性,提供未來乳癌新的預後及治療指標。本研究利用乳癌患者腫瘤組織檢體,以免疫組織化學染色探討SOX4, PLAU和PLAUR在腫瘤組織的表現,再與病人的存活狀況作統計分析。結果發現SOX4, PLAU和PLAUR在乳癌患者的腫瘤組織有不同程度的表現量,在正常組織中卻是幾乎沒有表現。統計分析結果顯示SOX4, PLAU和PLAUR的表現量相互有顯著的正相關。另外,SOX4與PLAU表現量和在不同病理分期乳癌腫瘤組織有顯著統計差異。乳癌患者中,SOX4高表現量且PLAU/ PLAUR表現量高的存活率較SOX4表現量低,且PLAU/ PLAUR表現量高者來的低;乳癌病人其SOX4、PLAU和PLAUR皆表現量過高者,存活時間最短。此外,SOX4和PLAU表現量與乳癌分子型:Luminal A和Luminal B有相關性;PLAUR則是與乳癌分子型無顯著差異。總結以上研究結果,SOX4、PLAU和PLAUR表現除了可單獨作為乳癌患者預後指標外,同時檢測三個指標更能準確預測乳癌病人的預後。
