Abstract
多形性星狀膠質母細胞瘤 (glioblastoma multiforme, GBM) 為成人中最常見的原發性惡性腦腫瘤,罹患後即使接受治療仍有一半的病人存活時間不及兩年,其復發率高且預後不佳。 Temozolomide (TMZ) 屬於口服烷化劑藥物,是臨床上治療 GBM 的第一線化療用藥,然而 GBM 對 TMZ 的抗藥性會造成治療上的困難,因此找尋有效的輔助藥物或許能為病人帶來一絲曙光。
Piperlongumine (PL) 是從長胡椒 (Piper longum) 萃取的活性生物鹼,文獻指出它具有治療癌症包括膠質瘤的潛力,但是 PL 毒殺 GBM 細胞的機制還未完全釐清,本篇研究目的為探討 PL 抑制 GBM 惡化所調控的網絡。首先,我們確認 PL 會誘導多種 GBM 細胞株的凋亡,相較在人類正常星狀膠細胞更具有毒殺效果,也觀察到 PL 能減少細胞侵犯、形成 colonies 和 spheres,以及增強 TMZ 的效果。將 PL 處理的 GBM 細胞進行 next-generation sequencing (NGS) 建立 PL 調控的 transcriptomic profile,並將 PL 抑制的基因、The cancer genome atlas (TCGA) 中相較正常組織在腫瘤大量表現的基因,以及三個不同的 GBM 資料庫裡的危險基因做交叉比對,得出 Tripatite motif-containing 14 (TRIM14) 可能參與在 PL 毒殺機制中,利用 TCGA 的 GBM 病人做存活率分析觀察到 TRIM14 表現量高者存活狀況確實較差,TRIM14 在 GBM 中的表現量相較在正常腦組織或細胞中高,代表它可能在 GBM 中扮演致癌基因的角色。PL 可顯著減少GBM 細胞中TRIM14的表現量,且 p38 MAPK pathway 參與在 PL 抑制 TRIM14當中。過量或抑制 TRIM14 表現會影響 GBM 細胞生長及 PL毒殺能力。最後透過 gene set enrichment analysis (GSEA) 分析觀察到和 TRIM14 呈正相關的基因與 epithelial to mesenchymal transition (EMT) 相關,並確認 TRIM14 參與在 PL 抑制 EMT 當中。本篇研究發現 PL 透過活化 p38 MAPK pathway 標的 TRIM14 誘導 GBM 細胞的毒性,揭示其對膠質瘤細胞更完整的毒殺機制,期許此論文能在日後治療 GBM 提供新的策略及方向。
