Abstract
背景:婦女遭受親密伴侶暴力(intimate partner violence,簡稱IPV)的現象,是全球普遍性的公共衛生議題,且嚴重威脅到婦女的基本人權。而受到長期內戰、武裝衝突影響的剛果民主共和國(Democratic Republic of the Congo,簡稱DRC)為全球IPV盛行率最高的地區之一。本研究旨在應用地理資訊系統(geographic information system,簡稱GIS)及多層次空間迴歸模型,來探討IPV於DRC的地理分布情形和集中趨勢,同時找出與當地婦女遭受IPV相關的重要個人、社區層級影響因子。
方法:本研究採橫斷式研究設計,數據資料來自2013到2014年剛果民主共和國人口與健康調查(Democratic Republic of Congo Demographic and Health Survey,簡稱DRC-DHS),總計有來自536個社區,5,210位年齡介於15到49歲的婦女被訪談並納入分析。使用克利金法(Kriging)和區域型空間自相關分析(Local Indicator of Spatial Association,簡稱LISA)來描述IPV的地理分布與集中趨勢,並建立多層次空間迴歸模型,分析與親密伴侶肢體暴力、情緒暴力、性暴力及任一形式暴力相關之危險因子。
結果:高IPV盛行率及群聚現象出現在DRC中南部區域之省份。親密伴侶肢體暴力為所有類型中盛行率最高者,接近45%的婦女曾經經歷親密伴侶肢體暴力,且約55%婦女曾遭受過至少一種形式的IPV。分析結果中,個人層級顯著危險因子包含:伴侶控制行為、婦女對於毆打的行為抱持贊同的態度、婦女的母親曾經歷過IPV、家中有較多小孩、伴侶有飲酒行為,以及婦女較年輕的初次結婚年齡;社區層級因子中則發現,社區中曾經歷過任一形式IPV的婦女比例與婦女遭受任一類型IPV的風險呈現顯著之正向關係。
結論:婦女遭受IPV在DRC是一個特別嚴重的議題,且可以發現IPV盛行率存在地理分布上的差異,一些中南部的省份特別需要相關介入來處理此問題,此外,不僅是個人層級因子,社區的背景脈絡同樣會影響婦女IPV的風險,因此,未來可針對此研究所找出的重要影響因子,來規劃執行IPV的預防計畫。
