Abstract
乾癬是一種慢性發炎性疾病,不僅影響皮膚,亦可能侵犯關節及心血管系統。外用類固醇為目前治療乾癬最常使用的藥物,但長期使用可能導致皮膚萎縮、產生抗藥性及停藥後反彈;而系統性治療則可能引發其他副作用。過去研究指出,間質幹細胞(mesenchymal stem cell, MSC)所分泌之外泌體(exosomes)在乾癬治療上具有潛在療效,顯示其外用應用可能成為輕至中度乾癬患者的另一種治療選擇。目前最常用於乾癬之MSC外泌體來自臍帶,但未了解外泌體中有效的乾癬治療成分為何?
過去未曾有關於胎盤來源外泌體對於乾癬治療之研究,本研究旨在探討胎盤來源外泌體於乾癬治療中的療效,並進一步尋找其可能的關鍵活性成分。由於微小核糖核酸(microRNA, miRNA)在乾癬的致病機轉中扮演重要角色,因此推測特定miRNA可能為胎盤來源MSC外泌體的主要作用分子。
首先利用imiquimod(IMQ)誘發之乾癬樣小鼠模型,驗證胎盤外泌體之治療效果。研究結果顯示,與未治療組相比,胎盤外泌體處理組具有顯著降低的表皮厚度、發炎性細胞因子及乾癬相關細胞因子(如介白素12、介白素17與介白素23)濃度。為了找到外泌體中可能有效的miRNA,進一步透過轉錄體分析乾癬動物模型及胎盤外泌體miRNA表現互相比對,發現miR-100-5p可能為胎盤外泌體的關鍵療效因子。再經過生物資料分析,得到miR-100-5p的可能預測靶點,其中以哺乳動物雷帕黴素標的蛋白(mTOR)及其下游激酶S6K1路徑於乾癬扮演的角色極為重要。於IMQ小鼠模型中,皮膚組織中磷酸化mTOR/S6K1顯著升高,使用胎盤外泌體則可使皮膚組織中磷酸化mTOR/S6K1降低;抑制胎盤外泌體中的miR-100-5p會削弱胎盤外泌體的治療效果且讓mTOR/S6K1顯著升高。此結果表示miR-100-5p透過調控mTOR/S6K1訊息傳導路徑達到療效。
過去文獻尚未對不同種類來源MSC外泌體療效進行直接比較。確定胎盤來源外泌體對乾癬的療效後,我們進一步比較胎盤外泌體、最常使用之臍帶外泌體及外用類固醇的治療效果。結果顯示胎盤與臍帶來源外泌體均能改善乾癬症狀,兩者在臨床嚴重度、表皮厚度、脾臟/體重比及細胞因子等方面的直接比較並無統計上顯著差異。於臨床嚴重度與表皮厚度改善上,兩種外泌體與外用類固醇效果相當,但在脾臟/體重比改善方面,外用類固醇優於胎盤外泌體,且在抑制介白素6 與介白素23方面也比兩種外泌體更為有效。
綜上所述,外用胎盤來源外泌體對於乾癬有療效,並經本研究證實miR-100-5p在胎盤外泌體治療乾癬的機制中扮演關鍵角色,可能透過調控mTOR/S6K1訊息傳導路徑發揮作用。胎盤與臍帶來源外泌體展現相當療效,未來仍需進一步研究以釐清其療效差異之分子機制並更有效增加其療效。
