Abstract
背景:
2013年台灣爆發了鼬獾狂犬病疫情,故遭鼬獾抓咬傷之民眾為感染狂犬病之高風險族群,且由於狂犬病之高致死率,因此鼬獾抓咬傷是一重要之公共衛生議題,故本研究乃欲探討遭鼬獾抓咬傷族群之流行病學特徵。
方法:
利用疾病管制署回溯性與前瞻性之動物抓咬傷監測系統資料,針對其中遭鼬獾抓咬傷之民眾以問卷進行電話訪問,收集個案之人口學資料、暴露時間、暴露地點、暴露情境以及暴露後醫療處置等資料進行整理分析。
結果:
92名遭鼬獾抓咬傷個案平均年紀為51±18歲,57名(62%)為男性,職業以從事農業相關行業(29名,32%)為最多。79%的個案於都市化程度較低之鄉鎮受傷(73人)。54%的個案因激怒行為而遭鼬獾抓咬傷,其平均年齡顯著低於非因激怒行為而遭鼬獾抓咬傷之民眾 (51 vs. 57歲),而男性(72% vs. 50%)以及上肢受傷(80% vs. 24%)之比例則顯著較高。僅47%之個案有適當地沖洗傷口(45人),而33%之個案未能依時程接種狂犬病暴露後疫苗。
結論:
台灣地區遭鼬獾抓咬傷之個案主要是成年男性,而受傷地點多在都市化程度較低之鄉鎮地區。約一半之個案是因激怒行為而遭咬傷。應加強民眾傷口清洗及按時程接種狂犬病暴露後疫苗重要性之認知。
