Abstract
背景:隨著平均壽命延長及人口老化,國人罹患慢性疾病比率逐年增加,心臟疾病、腦血管疾病、糖尿病、高血壓與慢性腎病等名列十大死因並造成社會經濟極大的損失,而高血脂為這些疾病的重要致病因子或共病症。多數研究探討高血脂的發生率或盛行率及其相關因子是以短期間進行資料收集與分析,少有研究觀察高血脂在不同性別、年齡的長時間趨勢變化,因此本研究以次級資料庫進行高血脂長期趨勢變化分析。
目的:
1.分析十年間臺灣北部某健康檢查機構受檢者高血脂盛行率之趨勢變化。
2.分析十年間臺灣北部某健康檢查機構受檢者不同性別及不同年齡層高血脂盛行率之趨勢變化。
研究方法:本研究使用台灣北部某健康檢查機構之健檢資料庫,擷取2013年至2022年接受血脂檢查民眾的健檢資料,總樣本數為288,274人,以年度百分比變化進行統計分析,並以性別和年齡分層,探討十年間高血脂盛行率的趨勢變化。
研究結果:整體族群僅高三酸甘油酯呈現統計上顯著逐漸下降趨勢 (APC為-3.525%,P<0.001)。
依性別分層,男性高血脂 (APC為-0.749%,P=0.020)與高三酸甘油酯 (APC=-2.180%,P<0.001)呈現逐漸下降趨勢;女性高血脂 (APC=2.004%,P<0.001)、高膽固醇 (APC=2.455%,P<0.001)、低密度脂蛋白膽固醇偏高 (APC=2.764%,P=0.019)呈現逐漸上升趨勢。
依年齡分層,高血脂於20-29歲 (APC=2.744%,P=0.003)、30-39歲 (APC=1.271%,P=0.011)及50-59歲 (APC=1.037%,P=0.019)呈現逐漸上升趨勢。高膽固醇於20-29歲 (APC=3.370%,P=0.004)、30-39歲 (APC=2.437%,P<0.001)、40-49歲 (APC=1.463%,P=0.022)及50-59歲 (APC=1.937%,P=0.004)均顯示統計上顯著的逐漸上升趨勢。高三酸甘油酯於30-39歲 (APC=-2.013%,P=0.002)、40-49歲 (APC=-2.344%,P <0.001)呈現逐漸下降趨勢;低密度脂蛋白膽固醇偏高於20-29歲 (APC=3.830%,P=0.008)及50-59歲 (APC=2.188, P=0.036)呈現逐漸上升趨勢。
結論:女性與較年輕的族群於高血脂、高膽固醇與低密度脂蛋白膽固醇偏高於十年研究期間有逐漸上升的趨勢。
