Abstract
為確保醫療體系能夠正常運作,醫事專業人員經常需要輪班工作,然而過往研究已觀 察到在輪班工作環境中可能會對認知表現與大腦功能造成危害。近期研究更證實輪班 工作引發的睡眠剝奪會改變腸道菌群組成與短鏈脂肪酸(SCFA)濃度,以及增加炎症 標記物,進而穿透血腦屏障,影響大腦功能,引起焦慮; 憂鬱及其他負面效應,降低 醫療人員對於病患的照護品質。然而,現階段學界仍不確定輪班工作所引發的節律失 調或睡眠剝奪等環境壓力如何衝擊腸腦軸線及其交互作用。故本研究旨在探索醫護人 員在輪班工作狀態下認知功能與腸腦軸線的動態變化,期望能更全面理解輪班工作對 於腸腦軸線的影響,進而提升醫事人員的生活品質和病患的照護品質。
我們於台北都會區招募十位輪班醫療人員進行追蹤式研究-在輪班周期前; 輪班後以 及休息期後進行了三個時間點的連續量測。測量項目包含(1)功能性磁振造影(fMRI) 以評估大腦功能性網路連結; (2)採集糞便樣本以確定腸道菌群組成和 SCFA 濃度,以 及(3)神經心理學評估。研究旨在分析腦功能; 腸道菌群; 和認知功能的變化趨勢;並 進一步運用相關性分析與中介分析以闡明彼此的交互關係。
結果顯示,輪班工作會降低執行控制功能的處理速度,如多任務處理,同時也改變了 大腦背側專注力與預設網絡的功能連結;然而,在腸道菌群和 SCFA 的變化間皆未發現 顯著改變。結果也顯示短期輪班工作或晝夜節律改變確實會改變大腦功能,但未足以 影響腸道菌相或其代謝物,但腸腦軸線的交互關係也會因輪班工作而產生變化。
