Abstract
過敏性氣喘(Allergic asthma)是一種常見慢性呼吸道疾病,主要由第二型輔助T細胞(T helper 2; Th2)所主導,在其分泌之細胞激素IL-4、IL-5、IL-13的作用下,會造成呼吸道上皮細胞受損、呼吸道過度反應(airway hyperresponsiveness; AHR)等。在先前的研究中發現介白質38具有抗發炎的功效,因此本篇論文想了解,介白質38對骨髓衍生性樹突狀細胞(bone marrow-derived dendritic cells; BM-DCs)的免疫調節作用以及在過敏性氣喘動物模式的治療潛力。實驗結果發現,重組蛋白介白質38(100 ng/ml)的作用能夠顯著減少促發炎細胞激素IL-1β、腫瘤壞死因子-⍺(TNF-⍺)以及IL-6的分泌,並且增加抗發炎細胞激素IL-10的分泌,此外,經重組蛋白介白質38作用後的樹突狀細胞會減少表面分子CD80、CD86以及MHC II的表現;另外,將事先經重組蛋白介白質38作用後的樹突狀細胞和異體性(allogeneic)CD4+T細胞共同培養後,能夠促進CD4+CD25+Foxp3+T 調節型 T細胞(regulatory T; Treg)生成,並且減少T細胞的增殖。接著,我們將腺病毒載體IL-38 (Ad-IL38)運用在卵白蛋白(ovalbumin; OVA)所誘導的過敏性氣喘動物模式上,和對照組相比,單次給予1×108 pfu Ad-IL38的組別,除了可以減少對OVA具特異性的IgE抗體產生、改善AHR外,還能夠減緩嗜酸性球在肺部的浸潤;而在肺沖洗液的分析中,給予Ad-IL38的小鼠組別,同樣能夠減少細胞激素IL-4、IL-5、IL-13、TNF-⍺以及IL-6的分泌,並且也減緩了肺部發炎細胞浸潤、黏液的產生,以及上皮細胞標記 IL-25、IL-33、TSLP 和黏液標記 Muc5ac的mRNA表達。最後,我們探討經由Ad-IL38感染的樹突狀細胞是否能夠用來治療過敏性氣喘小鼠。將經過Ad-IL38感染的樹突狀細胞(IL38-DC)給予過敏性氣喘小鼠,同樣能夠改善對OVA具特異性的IgE抗體產生、Th2型細胞激素的分泌、AHR和肺部發炎。值得注意的是,IL38-DC的治療增加了Foxp3+Treg細胞的數量,並且抑制了肺組織中 NF-????B 和 MAPKs 訊息傳遞路徑的活化。綜上所述,我們認為IL38-DC具有免疫調節能力,透過促進Treg的產生來抑制Th2免疫反應,可作為未來治療氣喘的新選擇。
