Abstract
思覺失調症是一個嚴重的精神疾病,在全球的盛行率約1-17‰,
而在台灣的盛行率則約6.4‰ 。思覺失調症會造成認知功能退化甚至失智症,特別是對於已接近或步入老年的患者更易造成認知衰退,其特徵比失智症更慢性、病程更久。過去國外研究發現思覺失調症可能是罹患失智症的風險因子,然而在台灣卻少有相關研究;本研究利用全民健保資料庫2005年百樣抽樣歸人檔為資料來源進行回溯性追蹤研究,發現有思覺失調症的病患相較於沒有思覺失調症的病患在經過潛在干擾因子校正過後,有顯著較高的失智症罹患風險,推論思覺失調症可能為罹患失智症風險因子。而年齡越大、共病越多者,失智症的發生率亦越高,在人口老化日益增加的台灣,失智症個案數想必也會越發增加;相對的,長照資源耗用於失智症的照護也會日益增加,故早期發現、早期治療思覺失調症,是否能避免病情惡化導致失智症的發生,對於長照資源花費於的節省有否幫助是未來研究的方向。
