Abstract
研究目的:以系統性文獻回顧與統合分析探討原發性腦腫瘤成年患者睡眠障礙之盛行率和危險因子。
研究設計:系統性文獻回顧與統合分析。
研究方法:於PubMed、Embase兩個資料庫中搜尋至2023年3月止之原發性腦腫瘤成年患者睡眠障礙之盛行率和危險因子的觀察性研究。文獻品質以Joanna Briggs Institute critical appraisal 之評讀標準進行評讀。以隨機效應模式分析資料以及利用Comprehensive Meta-Analysis Software 2.0統計軟體進行分析。使用Cochrane Q test 及I² test 檢測異質性。使用Egger’s test檢測是否有出版偏差。
研究結果:本文共納入24篇文獻,包含19篇為橫斷性研究、3篇為縱貫性研究,一篇為病例對照、一篇為回溯性研究。統合分析後結果顯示不良睡眠品質、失眠及白天過度嗜睡於治療前、後盛行率分別為:69.2%、53.3%、5.6%及40.4%、39.5%、44.6%。Egger’s test結果顯示,治療前、後不良睡眠品質、失眠和白天過度嗜睡的盛行率不具有發表偏差。統合迴歸分析結果,治療後失眠結果顯示,男性比例每增加一個百分比,失眠盛行率會增加6%,治療後白天過度嗜睡結果顯示,男性比例每增加一個百分比,白天過度嗜睡盛行率會增加11%,而原發性腦腫瘤成年患者睡眠障礙之盛行率不受國家與睡眠評估工具的影響。
結論:不良睡眠品質與失眠於治療後盛行率會低於治療前,而白天過度嗜睡於治療後整體盛行率則呈現明顯上升趨勢,原發性腦腫瘤成年患者睡眠障礙危險因子與頭痛及類固醇使用有顯著相關。期望本研究能提供臨床醫護人員日後在改善原發性腦腫瘤患者睡眠障礙介入措施之參考,進而增進健康相關生活品質。
