Abstract
本文主要是以12位確診、領有健保署核發有效重大傷病卡,且外觀看來與常人無異,不需輪椅或助行器來輔助行動的多發性硬化症病友為研究對象。受訪病友為7名女性、5位男性,散居台灣西部各城市,病齡短則數年,長則有26年之多。藉著病友的敘述,從而了解多發性硬化症在發病初期可能以「身體某部分產生異樣麻痺感」、「視力受到影響」、「無預警部分癱瘓」做為表現形式。對病人而言,初期對疾病一無所知,未能立刻產生憂懼的反應,需到確診、反覆發病後才會感受到疾病帶來的影響。然而對醫師而言,隨著診斷技術的發展,較過去已能更準確地及早診斷、及早開始治療。
綜合病友經驗,個體生命因疾病而有巨大改變,進一步產生生理、心理與社會三個層面的受苦經歷。生理層面的受苦包括多發性硬化症的各樣症狀,以及治療過程中的副作用。心理層面的受苦包括需要經歷「悲傷五階段歷程」,以及面對疾病的未知而有各種害怕和擔心。社會層面的受苦包括人際關係的限縮,以及職涯發展上的變化。綜合這三方面的多重衝擊,則可能讓病友有自殺的意念或行動。
此外,病友也會因男性或女性的生理性別差異,在職場、家庭與生育的議題上有不同的生病經驗。病友敘述女性在職場上能得到較多的包容與協助,對男性在職場上仍有一定的工作表現要求。相對的,因華人傳統角色觀念認為,女性應以家庭做為最優先的順位,以致女病友在病後仍會繼續負擔原有的照顧角色,包括母親照顧兒女,以及女兒奉養父母。在生育議題上,女性病友明顯對此有較多的關注和討論。對病人而言優先條件是要在不用藥的情況產下健康的寶寶,其次才是希望能避免發病、有足夠的體力和精神哺養孩子。
面對疾病帶來的各種影響,病友終將找到與病共處的策略,包括:(1)「避免太熱的環境」、(2)「避免過高的壓力」、(3)「避免過分勞累」、(4)「尋求病友團體或週遭他者的扶持」。據此病友會調整作息與活動模式,也會放棄完美主義、放棄緊湊行程、放棄對成就的追逐。從常人的眼中來看,這些改變或許是一種失敗,但對病人來說,避免發病才是病人們終其一生需要面對的課題。要學會感受身體在遭遇不同情境時所發出的訊號,從中得到經驗,知道如何免除發病、適時尋找幫助,進而達到和疾病和平共處的境界,讓自己能在安全的情況下帶病生活。
