Abstract
研究背景與目的:非酒精性脂肪肝為肝臟發生脂肪變性,常與許多代謝性疾病合併發生,並造成腸道菌相失衡、循環膽酸組成份改變及成纖維細胞生長因子19表現量降低。過去研究發現凝結芽孢桿菌具有降低肝臟脂肪堆積及腸道菌相失衡的問題。但對於非酒精性脂肪肝疾病患者補充凝結芽孢桿菌之影響尚未確認。故本研究目的為探討補充凝結芽孢桿菌TCI711益生菌對於非酒精性脂肪肝疾病患者肝臟脂肪堆積、膽酸-類法尼醇X受體-成纖維細胞生長因子路徑與腸道菌相之影響。方法:57位被診斷為非酒精性脂肪肝疾病的患者,採隨機雙盲安慰劑對照試驗之設計,所有實驗參與者分別服用8週含有1 x 1010 colony-forming unit (CFU) 的益生菌或安慰劑 (每日一次),於每四週回診一次,評估其順從性、問卷調查、體位及血壓測量,並在補充前 (第0週) 與補充後 (第8週) 進行血液與糞便的採集及肝纖維儀檢查。結果:進行補充前,兩組組間數值並無顯著差異;TCI711組在補充8週凝結芽孢桿菌TCI711益生菌後,肝纖維儀檢查結果顯著低於補充前 (p=0.023);安慰劑組血漿中成纖維細胞生長因子19水平在補充後顯著高於補充前 (p=0.045);同時,在腸道菌相組成方面,觀察到凝結芽孢桿菌TCI711益生菌可能透過降低腸道中病原菌的產生及炎症反應、穩定腸道代謝物的組成與改善膽酸代謝失衡的方式,而具有維持腸道菌相組成的益處。結論:非酒精性脂肪肝疾病患者連續補充8週凝結芽孢桿菌TCI711益生菌後能有效降低肝臟脂肪堆積,可能與此益生菌具有穩定腸道菌相組成的效果有關;然而,凝結芽孢桿菌TCI711益生菌是否經由膽酸-類法尼醇X受體-成纖維細胞生長因子路徑,影響非酒精性脂肪肝疾病患者肝臟脂肪堆積的狀況,仍有待未來研究進一步釐清。
