Abstract
肝癌依期別與病患個別性有多樣性的治療方式,早期或沒有肝功能不良的病人,治療方式仍以手術切除為優先考慮,但大部分的病人發現已晚或嚴重肝炎肝硬化、肝功能不良等無法手術切除或移植或做過各種治療後復發,此時,射頻燒灼術(Radiofrequency ablation; RFA)成了治療早期肝癌或等待手術移植期間有效且重要的治療方式。肝癌治療後的高復發率,仍是很重要的問題,早期易復發與否往往影響到病人長期的存活率,對於等待肝臟移植醫療規劃的病患,由於器官數量有限,允許等待的時間也很重要。故此研究希望能透過類神經網路預測模型,以仿效生物神經網路方式分析非線性資料,並利用肝癌有關的多變數因子,來提供準確的預測執行射頻燒灼術後肝癌病患早期(一年、二年)無病生存期。
此研究屬回溯性研究,追蹤2009年1月至2012年4月利用電腦斷層導引經皮射頻燒灼術治療肝癌之病患,收集第一年無病生存病患共252人,第二年無病生存病患共179人,分別納入15項、8項和6項與肝癌和治療有關之輸入變項。以內部預測回溯性資料之驗證方式與外部預測模擬前瞻性資料之驗證方式,共建構八種類神經網路預測模型並分析與評估其模型之效能。
結果顯示輸入15項變項具有較好之預測力,第一年無病生存期之內部預測回溯性資料方式,其內部測試驗證組預測效能準確度85.0%、敏感度75.0%、特異度87.5%、AUC0.84與外部測試之驗證組準確度70.0%、敏感度63.6%、特異71.8%、AUC0.77皆具有良好之預測效能。而第二年無病生存期之內部預測回溯性資料,內部測試驗證組預測效能準確度67.9%、敏感度50.0%、特異度85.7%、AUC0.75與外部測試驗證之預測模型準確度63.9%、敏感度56.3%、特異度70.0%、AUC0.72預測效能相當,第二年雖為較少訓練樣本但仍具有中度之預測力。
此次研究藉由人工智慧的資料探勘技術,調整執行的參數以找出更好的組合,這項研究顯示所開發之預測模型具有中度以上的預測能力,可提供給臨床醫師作為輔助工具,以增加臨床決策和預後評估的方法。
