Abstract
已知乳癌婦女接受輔助性化學治療後 (adjuvant chemotherapy;簡稱輔助性化療) ,普遍發生體重、體脂肪增加之副作用,體重增加不僅造成乳癌婦女心理壓力,社交困擾,也提高罹患慢性病之風險,例如:糖尿病、高血壓和心血管疾病等,甚至影響生活品質,降低存活率。因此,本研究目的在探討乳癌婦女接受輔助性化療前,營養師予以介入營養教育,對其治療後之體重與體脂肪變化等營養狀況改善的影響。研究對象為18~65歲,由國泰綜合醫院一般外科、血液腫瘤科,新診斷並計劃接受輔助性化療之早期乳癌婦女。於乳癌病人接受化學治療前,以綜合問卷蒐集背景資料,包括:年齡、職業、學歷和活動程度;進行體位測量,測量項目包括:身高、體重、身體質量指數 (Body Mass Index, BMI)、體脂率、內臟脂肪;抽取空腹血液樣本,檢驗生化數值包括:總膽固醇、三酸甘油酯、低密度脂蛋白膽固醇、高密度脂蛋白膽固醇、糖化血色素,再由營養師詢問進行24小時飲食回憶,評估過後給予個人化營養介入以及營養教育。研究結束時,再次測量體位和檢驗上述生化數值,作為成效評估項目。本研究共17位受試者完成研究,相關數值經統計分析,結果發現,輔助性化療結束後,病人的體重、體脂率維持,內臟脂肪顯著下降,血液生化數值中,總膽固醇、低密度脂蛋白膽固醇和糖化血色素顯著降低;顯示輔助性化療期間,由營養師給予早期乳癌病人營養教育,對其輔助性化療後,體位維持不增加,有正面助益。
