Abstract
臨床抗癌藥劑CPT-11(irinotecan hydrochloride)為廣泛使用於各種惡性腫瘤之化療藥劑;然而腸道內大腸桿菌的β-glucuronidase(E coli. βG, eβG),會恢復CPT-11的毒性,造成嚴重腹瀉的副作用(chemotherapy induced diarrhea, CID)。若能抑制eβG之活性,將可以大幅降低CID副作用、突破劑量的限制以提升腫瘤治療療效。為達成此目的,我們已建立一套完善的活體內外eβG活性分析平台,可用於評估:何種中草藥萃取物可抑制腸內菌的eβG酵素之活性並能夠有效減緩CID並且提升其抗癌療效;研究結果發現:中草藥萃取物silymarin、Glycyrrhizin、以及對照組藥物D-Saccharolactone均可抑制eβG酵素之活性、均可進入菌內抑制eβG活性,並以silymarin抑制效果最佳;在抑制humanβG (hβG)實驗中,silymarin及Glycyrrhizin均不會抑制hβG酵素活性,然而D-Saccharolactone則會顯著抑制hβG活性;以螢光造影系統評估silymarin結果能有效抑制老鼠的腸內菌eβG之活性;評估silymarin作為CPT-11化療佐劑治結果證實能有效保護老鼠腸道免於血便並能快速從化療受的傷害恢復。本研究篩選出合適的化療佐劑silymarin,解決CPT-11化學治療所誘發嚴重腹瀉而停藥的問題,而得以進行最佳化之治療模式作治療。
