Abstract
背景:失智症為高齡社會中最具挑戰性的神經退化性疾病之一,其特徵為記憶、語言與執行功能的持續惡化,進而影響日常生活能力。日間照顧中心提供臺灣長照體系中的非藥物介入服務,透過結構化課程、認知刺激與社會互動,被認為具有延緩失智症認知功能退化的潛力。然而,國內針對失智症長者在接受日間照顧服務後的「長期縱向認知軌跡」研究仍有限,對於不同人口學特徵與基線功能狀態如何影響認知變化軌跡亦缺乏實證數據支持。
目的:本研究探討日間照顧中心之失智症長者在為期 12 個月服務期間的認知功能變化,並分析人d口學特徵與基線功能狀態是否影響認知退化的速率與模式,以釐清不同族群在日照服務下的認知軌跡的異質性。
方法:本研究採縱向觀察性研究之次級資料分析,納入 33 位輕度至中度失智症長者。於基線、第 3、6、9 與 12 個月共五個時間點,使用阿茲海默症認知評估量表 (Alzheimer's disease assessment scale -cognitive subscale,ADAS-Cog) 評估認知功能。研究運用廣義估計方程 (Generalized Estimating Equations) 分析認知功能隨時間的變化趨勢,並檢驗人口學特徵和基線功能狀態與時間的交互作用。
結果:結果顯示,整體樣本的 ADAS-Cog 分數在 12 個月期間顯著上升 (p < .05),表示認知功能隨時間呈現退化趨勢。此外,教育程度顯著調節認知的退化速率(交互作用 p = .016),呈現高度異質軌跡。研究進一步發現第 6 個月為認知軌跡的關鍵分歧點:較低教育程度長者呈現最早且最陡峭的退化;中等教育程度長者展現最佳的認知可塑性,前期維持相對穩定甚至改善;較高教育程度長者則在初期持平,但於中後期 6 至 9 個月出現加速退化現象。
結論:本研究為失智症長者在日間照顧中心的縱向認知變化提供實證,顯示認知退化具高度異質性,而教育程度是決定退化軌跡與速率的關鍵調節因子。建議日間照顧中心可以「六個月」間隔作為評估成效與調整照顧計畫的時間區段。
