Abstract
近年來研究利用腫瘤生物標記來早期診斷肺腺癌已是主流趨勢,而設法提高其特異性為首要目標,先前已有研究指出在肺癌患者血液或組織樣本中,α-1 抗胰凝乳蛋白酶(alpha-1-antichymotrypsin, AACT)的表現量有上升的現象,我們認為此蛋白有潛力成為肺腺癌的特異性腫瘤生物標記,此研究中我們以西方墨點法(Western blot)實驗得知肺腺癌患者血液檢體與α-1 抗胰凝乳蛋白酶表現量上升有高度的相關性(18 例中有16 例, 88.9%);雖然在健康者檢體中偶有發現微量表現(18 例中有1 例)。α-1 抗胰凝乳蛋白酶表現量上升的情況在晚期肺腺癌病患檢體中較為明顯,但在許多疾病中,α-1 抗胰凝乳蛋白酶表現量都有上升或下降的現象,故其做為篩檢工具特異性略顯不足,除了蛋白表現量差異,α-1 抗胰凝乳蛋白酶上與癌症相關的轉譯後修飾(posttranslational modification,PTM)可能也會影響其在癌細胞中的生理功能,故我們對其轉譯後修飾進一步分析,利用免疫沉澱法(Immunoprecipitation, IP)及奈米級液相層析串聯質譜(Nano-LC-MS/MS)分析比較肺癌患者與健康者檢體中α-1 抗胰凝乳蛋白酶的轉譯後修飾,最後有差異的轉譯後修飾(PTM)結果。
由上述實驗結果我們認為α-1 抗胰凝乳蛋白酶的甲基化轉譯後修飾有潛力做為診斷肺腺癌的特異性腫瘤生物標記,蛋白質表現量與轉譯後修飾對其蛋白生物功能性的影響以及與腫瘤侵襲或轉移的關聯性值得進一步研究討論。
