Abstract
急性肺損傷 (acute lung injury, ALI) 是一種以急性發炎反應和肺功能障礙為特徵的病理狀態。Glutamine (GLN)為條件性必需胺基酸,具有抗發炎、抗氧化以及器官保護作用。本實驗目的為探討靜脈給予GLN對poly(I:C)誘導ALI小鼠之影響。使用C57BL/6雄性小鼠,分為控制組 (Normal control, NC)、假手術組 (Sham group, SH)、poly(I:C) + 生理食鹽水組 (PS)以及poly(I:C) + GLN組 (PG)。以氣管注射每公斤體重3 mg之poly(I:C)誘發ALI,之後立即給予靜脈注射每公斤體重0.75 g之GLN或同體積之生理食鹽水。假手術組不進行氣管注射或靜脈注射。所有SH、PS、PG組之小鼠於氣管注射4、12或24 h後犧牲。結果顯示,poly(I:C)刺激後血漿中GLN、arginine以及支鏈胺基酸濃度顯著下降,促發炎因子。肺泡沖洗液中干擾素λ3濃度、巨噬細胞及M1巨噬細胞百分比/數量下降,嗜中性白血球百分比/數量、促發炎因子濃度及肺臟組織myeloperoxidase (MPO)活性及malondialdehyde (MDA)含量上升。此外,白血球遷移相關因子基因表現量增加,而肺泡-微血管屏障相關因子mRNA表現量下降。GLN補充增加TLR3抗病毒路徑及肺泡-微血管屏障相關因子基因和/或蛋白質表現量,同時降低促發炎因子、白血球遷移相關因子表現量、MPO活性及MDA含量。組織學分析亦顯示減緩肺損傷情形。顯示poly(I:C)刺激後補充GLN能維持血漿GLN濃度,且透過活化TLR3抗病毒路徑、抑制白血球遷移、嗜中性白血球浸潤、發炎反應、氧化壓力,以及改善肺泡-微血管屏障完整性以緩解 ALI。
