Abstract
睡眠障礙在全球人口中普遍存在,睡眠障礙與多種合併症有關。皮質醇經常被作為丘腦-垂體-腎上腺 (HPA) 軸失調的生物標記物,從而引發隨後的睡眠障礙。皮質醇和睡眠對健康的重要性以及兩者之間的雙向關係已被確定。HPA 軸是 CNS 調節免疫系統的主要途徑之一,過去文獻較多探討皮質醇與免疫系統的相關性。在睡眠障礙病人的皮質醇與炎症之相關研究文獻不足,故運用使用鎮靜安眠藥物住院病人的相關研究是重要的。
本研究以東部某醫學中心病歷資料庫為樣本,以使用鎮靜安眠藥物及未使用鎮靜安眠藥物之住院病人,有檢驗皮質醇及炎症指標之項目為研究對象,以描述性統計、卡方檢定、T檢定(T-Test)、皮爾森檢定(Pearson Correlation)及多元線性迴歸分(Multiple regression analysis)進行資料分析。研究結果發現:(1)有使用鎮靜安眠藥物與未使用鎮靜安眠藥物之住院患者的CRP、WBC及NLR的炎症指標與皮質醇是有顯著正相關(P<0.05及P<0.001),Lymphocyte、Monocyte、Basophil、Eosinophil、Platelet與皮質醇是有顯著負相關的(P<0.05及P<0.001)。(2) 在使用鎮靜安眠藥物與未鎮靜安眠藥物之住院病人,發現抽血時間06:00-09:59、10:00-13:59、14:00-17:59(P <0.001)及藥物種類Non-BZD(P<0.05)對皮質醇有顯著影響;另外年齡、性別、共病症、抽血時間02:00-05:59、14:00-17:59、藥物劑量、藥物種類中Non-BZD及BZD &Non-BZD(P<0.05)對炎症指標顯著影響。(3)以多元線性迴歸分析具有達顯著的影響因子之變項,進行t檢定分析發現使用鎮靜安眠藥物的患者在炎症指標較於未使用鎮靜安眠藥物結果為 WBC、Neutrophil、Monocyte及NLR(P<0.05)呈現顯著差異。研究結果為使用鎮靜安眠藥物住院病人之皮質醇與炎症指標具有相關性,發現皮質醇與炎症指標的影響因子,且部分具有顯著差異,希望提供於使用鎮靜安眠藥物患者之皮質醇與炎症影響,並從全血球細胞計數中評估血球比例的臨床意義以做參考。
