Abstract
研究背景
胺碘酮引起的甲狀腺功能障礙是一種危及生命、不可預測且常見的藥物不良反應。只收集單次病人資訊進行統計分析無法得到良好的預測結果,同時大資料庫分析常會遇到資料不平衡的挑戰,準確的機器學習能夠能良好預測此多重因子相關且複雜的藥物不良反應。
研究目的
以再採樣方法調整不平衡數據,以預測胺碘酮引起甲狀腺異常的風險,並藉由重新調整臨床決策閾值來優化機器學習模型。
研究方法
使用北醫臨床數據庫於2013年至2017年使用胺碘酮的病人進行研究,以來自臺北醫學大學附設醫院、萬芳醫院資料為訓練組,及雙和醫院為外部驗證組。本研究由4種機器學習演算法XGBoost、AdaBoost、KNN和LR模型,除原始數據外,並進行三種重新取樣方法,共建立16個機器學習模型。使用準確率、精確度、召回率、F1 分數、幾何平均數(G-mean)、ROC曲線及其面積和PRC曲線下面積來比較16種模型的表現,決定最佳模型後,再探討重新調整臨床決策閾值比較模型表現差異,決定於臨床使用的最佳閾值。
研究結果
本研究使用4075位病人進行訓練和2422位病人外部驗證,其中583(14.3%)為甲狀腺功能障礙組。外部測試集是來自雙和醫院2,422中有275(11.4%)發生甲狀腺異常。XGBoost-SMOTE在所有16個模型中展示了最好的預測結果。準確率0.909、精確率0.574、召回率0.764、F1分數0.660、ROC曲線下的面積0.926及PRC曲線下面積0.73,而最佳臨床決策閾值為0.673,在此最佳切點之下,F1分數可達最大值0.684。
結論
機器學習模型搭配重新取樣可以作為胺碘酮引起的甲狀腺功能障礙的個體化風險分層臨床決策的支持工具。
