Abstract
目前全球肥胖盛行率不斷上升,造成非酒精性脂肪肝病(nonalcoholic fatty liver disease, NAFLD)之風險逐漸上升。目前針對病態性肥胖患者減重手術後肝臟病理變化研究有限。本研究利用病態性肥胖大鼠模擬罹患非酒精性脂肪肝的患者,探討減重手術及飲食對血液、肝臟病理、脂質代謝與自噬的影響。本研究使用20隻8週齡雄性Wistar大鼠進行14週高脂飲食誘導肥胖,於22週齡進行減重手術,並探討術後持續高脂飲食對脂肪肝影響。於術後飲食介入並分別於第6及12週犧牲取得血液及肝臟檢體。結果顯示高脂飲食(HFD)組的體重與肝臟重量明顯高於控制組。在血液生化方面,飲食誘導後,HFD組血清ALT和AST顯著升高。術後12週,HFD組的ALT和AST均顯著低於術前;H&E染色及油紅染色結果同時顯示HFD組油滴明顯變多,而進行手術後,可觀察到油滴大幅度減少。此外,高脂飲食誘導後圍脂滴蛋白顯著增加,且脂質分解作用減弱與肝臟自噬作用下降。相反地,手術後圍脂滴蛋白顯著下降,同時脂質分解作用明顯提升,且脂質自噬作用也顯著增加。綜上所述,減重手術可改善病態性肥胖大鼠的肝臟脂肪含量和肝臟生化代謝功能,進而延緩甚至逆轉非酒精性脂肪肝病進程。
