Abstract
肌少症( Sarcopenia)是一種與老年人跌倒與骨折風險密切相關的病症,其特徵為骨骼肌質量與力量的進一步減退,常伴隨結締組織增生、肌肉脂肪化、慢性發炎、粒線體功能障礙及荷爾蒙失衡等病理現象。近期研究指出,腸道菌群失調可能在肌少症的進程中扮演重要角色,而益生菌如: Lactobacillus casei Shirota(LcS)具潛力調節菌群組成與衍生代謝物,進而影響宿主免疫、抗氧化機制與營養吸收。本研究旨在探討每日補充含 LcS 的發酵乳對台灣老年肌少症患者之腸道微生態、微生物代謝產物(如: IPA、SCFA、TMAO)與重金屬累積之影響,並評估其對肌肉功能與身體組成之潛在改善效果。研究設計採公開標籤單組前後比較,為期12週,招募40位符合AWGS標準之老年肌少症受試者。結果顯示,LcS補充顯著改善肌少症狀包括:握力、步態速度與下肢功能,並增加 GPx 活性、降低 CD4+ T 細胞比例與多種重金屬濃度(包括Pb、Cu、Zn等),初步支持腸道菌群—代謝物—肌肉功能之交互作用機制。此結果突顯 LcS 發酵乳在延緩肌少症進展與重金屬解毒方面的潛力,值得進一步探討其應用於健康老化之整合策略。
