Abstract
研究發現高胰島素血症及胰島素抗性對於老年人認知功能退化是重要的危險因子,另外也發現在老年人糖尿病控制不佳亦會加速認知功能退化,因此本研究於衛生福利部基隆醫院內分泌科門診進行收案,探討第2型糖尿病患若接受營養教育介入後血糖控制及認知功能改變是否有正向影響。本研究為1年介入性研究,招募對象為衛生福利部基隆醫院內分泌科50至70歲且罹患糖尿病年齡至少10年以上之第2型糖尿病患,依是否接受糖尿病衛教分成實驗組及對照組,依據系統抽樣,共招募26名及12名。介入前及介入後皆分別進行3日飲食紀錄、飲食行為及認知問卷調查、生化數值蒐集及認知功能測驗(Mini-Mental Status Examination, MMSE)。實驗組經過1年糖尿病衛教介入,包含營養諮詢、團體衛教及個別指導。結果顯示:介入後兩組的生化數值(糖化血色素、空腹血糖、總膽固醇、三酸甘油酯、高密度脂蛋白膽固醇及低密度脂蛋白膽固醇)、熱量攝取及MMSE分數皆無顯著差異,但飲食認知問卷分數實驗組顯著較對照組高(p=0.01),並發現介入後實驗組之糖化血色素、空腹血糖、總膽固醇及低密度脂蛋白膽固醇與MMSE分數相關性呈現顯著負相關(p=0.005、0.059、0.01、0.03)。結論:給予糖尿病衛教介入可顯著改善飲食認知觀念,有效控制血糖及血脂與改善認知功能有正向相關。
