Abstract
我國行政院主計總處國情統計通報資料指出,台灣平均每年有2萬起突發性心跳停止案例。臨床上經由腦部斷層掃描,測量大腦中灰質和白質之間組織密度的比例Grey Matter and White Matter Ratio(GWR),可用以定量化腦部水腫的程度,也可以藉以評估腦部在心跳停止期間所受到的傷害。研究顯示GWR用以預測神經學預後的數值可能會因為地域或族群不同而有不同的標準,故建立台灣族群神經學預後及存活狀態的模預測模型,為刻不容緩的目標,以提供預防機制及改善突發性心跳停止之治療方式與流程。
研究目的:本研究主要建立,適用台灣民眾之突發性心跳停止成功復甦後病患神經學預後及存活狀態預測模式。
研究方法:採用二手資料進行研究分析,資料來源為樣本醫院急診室,以病歷回溯性研究(Retrospective Study)的方法進行資料收集;研究對象為2006年到2015年間,發生非創傷性心跳停止經心肺復甦急救成功恢復持續自發性循環之病患;藉由神經學預後大腦功能分類 (Cerebral Performance Category, CPC)及出院存活狀態相關資料,分析其相關因子與神經學預後及存活之相關性,以建立神經學預後及存活之預測模式。
研究結果:本研究建立之神經學預後模型中納入最終模型之變項為年齡、初始心律、院內使用腎上腺素、格拉斯哥昏迷指數之運動反應、鎮靜藥物之使用;存活狀態模型中納入最終模型之變項包括年齡、心律不整、惡性腫瘤、初始心律、院內使用腎上腺素、低溫治療、格拉斯哥昏迷指數之運動反應、鎮靜藥物之使用及GWRave,模型經過Bootstrap重覆抽樣加以驗證後分別為C=0.80與C=0.77,皆呈現好的的鑑別力。
結論:美國及韓國,有多數以腦部CT之GWR來做為心跳停止病患之神經學及存活預後評估,並有良好預測力,相對國內目前並無此類研究。在本研究也發現發生突發性心跳停止年齡為重要的預測因子,隨年齡增加其神經學預後及存活愈不佳,對於逐漸邁入老年化的社會,建立預後預測模式之重要性更是當務之急,以利突發性心跳停止復甦後的急救措施提供更多訊息,進而使病患多一線生存機會。
