Abstract
研究背景與目的:人體腸道菌相會受到西方飲食、不良生活方式、壓力和衰老等因素影響,導致菌相失衡。長期菌相失衡可能會造成代謝症候群、肥胖和糖尿病等多種疾病。飲食是改變腸道菌相主要的方式之一,補充益生質會促進特定的益菌生長和增加代謝物短鏈脂肪酸濃度,並抑制病原菌生長。木寡醣作為一種新興的益生質,對於健康成人整體腸道菌相之影響仍需要研究。本次研究欲探討健康成人額外補充含木寡醣綠茶飲品四週後,是否能改善腸道菌相,增加腸道菌代謝物短鏈脂肪酸濃度,具有促進整體健康、預防疾病的作用。並以統合分析方法整合本次研究結果與過去之研究。了解益生質補充對於健康成人腸道菌相之作用。實驗設計與方法:本研究分為兩部分,第一部分為隨機雙盲對照交叉試驗。招募年齡介於20-65歲之健康成年人作為受試對象。實驗為期12週,共有兩次4週介入期,兩次介入期之間有2週排空期,試驗組於介入期每天攝取7.2克木寡醣。試驗期間測量體位、抽血、忝寫腸胃狀況問卷及三日飲食紀錄。並在每次介入前後收集糞便檢體分析腸道菌相及腸道菌代謝物短鏈脂肪酸。第二部分為統合分析研究。根據PICOs設定搜尋策略,建立關鍵字(P: healthy adults; I: prebiotics; C: placebo; O: gut microbiota AND short chain fatty acid)蒐集2000年至2021年6月前國內外研究以及第一部分試驗結果進行統合分析。結果:經過四週木寡醣介入,Bifidobacterium顯著增加(109.4 ± 101.6, 1010.4 ±101.1; p< 0.001);整體菌相觀察到F/B ratio顯著下降(0.74 ± 0.3854, 0.42 ± 0.1516; p< 0.05),益菌增加,病原菌減少;總短鏈脂肪酸含量上升(23.17 ± 4.18, 34.55 ± 13.23; p< 0.05),其中乙酸濃度顯著增加(22.71 ± 6.93, 27.50 ± 10.87; p< 0.05)。統合分析共納入了15篇文獻,結果顯示益生質的補充會使健康成人腸道中Firmicutes相對豐富度顯著減少(MD:-6.80;95% CI:-13.51, -0.10,I2= 0%),顯著促進Bifidobacterium生長(MD:0.72;95% CI:0.34, 1.10, I2= 47%),在短鏈脂肪酸方面會顯著增加丁酸濃度(MD:2.49;95% CI:0.20, 4.77,I2¬= 82%)。結論:健康成人攝取益生質可使腸道中益菌增加,抑制壞菌生長,並增加丁酸濃度,具有改善整體健康的作用。
