Abstract
當病患因為生理或疾病需求而使用呼吸器超過21天稱為「長期使用呼吸器的病患」。此現象若發生在加護病房時須轉至呼吸照護中心(respiratory care center, RCC)訓練脫離呼吸器。而使用呼吸器的重症病患,易發生營養不良的問題。營養不良會引起呼吸肌肉收縮變差、換氣能力降低,呼吸耐力降低,影響呼吸機制,而延長使用呼吸器時間,甚至容易增加感染率、致病率、延長住院天數及死亡率。本研究目的擬觀察成功脫離呼吸器及呼吸器依賴患者間各相關因素,包括:呼吸器脫離指標、血清白蛋白、熱量及蛋白質攝取、身體肌肉組成的差異。本研究共招募62位佛教慈濟醫療財團法人台北慈濟醫院呼吸照護中心(respiratory care center, RCC)受試者,43位成功脫離呼吸器、19位呼吸器依賴,成功脫離呼吸器比率為54.8 %。研究結果顯示,成功脫離呼吸器的相關因子包含:受試者入住RCC的天數,成功脫離呼吸器組23.1 ± 11.1天顯著低於呼吸器依賴組35.6 ± 7.8天(P<0.05);PImax部份,成功脫離呼吸器組-27.09 ± 9.7 cmH2O顯著大於呼吸器依賴組-21.4 ± 10.2 cmH2O(P<0.05);疾病嚴重度APACHE Ⅱ,成功脫離呼吸器組15.8 ± 5.0分顯著低於呼吸器依賴組20.4 ± 8.4分(P<0.05)。受試者血清白蛋白濃度,成功脫離呼吸器組2.5 ± 0.4 mg/dL與呼吸器依賴組2.5 ± 0.4 mg/dL相較,未達統計差異,但在成功脫離呼吸器組中前、後測比較,受試者的血清白蛋白濃度由2.2 ± 0.3 mg/dL顯著增加為2.5 ± 0.4 mg/dL(P<0.05)。結論,營養狀態的改善,能幫助RCC患者成功脫離呼吸器。
