Abstract
研究背景:生育是女性重要的生命歷程,婦女在懷孕與生產過程,經驗許多身體與心理變化。正向的生產經驗可以促進母嬰關係,而負面生產經驗可能會影響女性的產後適應與家庭功能,甚至導致產後憂鬱症狀產生。台灣婦女生產經驗與產後憂鬱的關係,為產後照護重要健康議題。
研究目的:探討生產經驗、連續性照護、憂鬱症狀的影響因素,並分析三者之關係。
研究方法:本研究採橫斷式研究設計,納入條件為年齡18歲以上、5年內具生產經歷的婦女、能夠說和讀中文、願意參與並配合研究流程。排除條件為具國外之生產經驗的婦女。
研究工具為基本資料表、產科資料表、生產經驗表、與產後憂鬱症狀量表。資料以收集線上方式進行收案,在母嬰相關網站等進行收案。統計分析以描述性統計與推論性統計,包括獨立樣本t-檢定、單因子變異數分析與卡方檢定、多元迴歸分析進行分析。
研究結果:本研究共收案303位產後婦女,研究對象主要集中於30至39歲之間。根據卡方檢定,生產經驗正向組產婦以30-39歲居多(p=.04),自然產者易出現負向的生產經驗(p=.01),且於醫院生產者較容易產生負面生產經驗(p=.001)。婦女的生產經驗與產後憂鬱之間具有顯著關聯(p = .034),根據迴歸分析指出,而產程介入措施(p = 0.041)、連續性照護(p = 0.035)對產後憂鬱的影響尤為明顯。
結論及討論:生產經驗、連續性照護程度與產後憂鬱之間呈現顯著關聯,婦女生產過程中的參與程度,與產後心理健康狀態有關。未來臨床照護強調生產過程的參與度,提升連續性照護可有助產後心理健康與適應。
