Abstract
冠狀動脈疾病(Coronary artery disease, CAD)是全球死亡原因第一位。經皮冠狀動脈介入治療(Percutaneous coronary intervention, PCI)是重要的治療方式,為預防支架血栓的發生,病患通常需配合服用抗血小板凝集藥物。由於心臟病患者常合併有高膽固醇、高血脂,故多會購買調整血脂或促進血液循環的健康食品來增加健康,而心臟病患者在接受PCI治療後需服用抗血小板藥物,若同時服用具抗凝血作用的健康食品,可能造成出血的傾向。因此本研究目的在探討服用健康食品對於接受PCI治療出血傾向及影響因素。
本研究共收案131位個案,採問卷調查法,以台北市某醫學中心心臟內科病房,安排入院接受PCI治療患者為試驗對象,並依個案是否服用健康食品分為服用組(n=39)及未服用組(n=92)兩組,共進行三階段收集資料,第一階段:入院時以基本資料表、服用健康食品調查表、出血評估量表問卷、PCI治療後進行心導管術後穿刺部位傷口評估表,第二階段:於一周後門診追蹤心導管傷口評估及評估個案健康食品服用狀況,第三階段:於三個月後以基本資料表、服用健康食品調查表、出血評估量表問卷進行資料收集。
結果顯示,個案中男性居多佔74.8%,平均年齡66.9(±12.1)歲。兩組在心導管傷口出血的比較結果顯示,服用健康食品組當日出現滲血與瘀青之比例高於未服用健康食品組(X2=9.379,p=0.009)。PCI後三個月服用健康食品組在皮膚出血(0.49 vs. 0.18 , p < 0.05)、口腔內出血(0.31 vs. 0.05 , p < 0.05)和整體性出血平均分數(0.10 vs. 0.05 , p < 0.05)皆顯著高於未服用健康食品組。另外在全身性出血情形發現服用健康食品組,除在皮膚出血得分上PCI三個月後顯著高於PCI後立即之分數(0.49 vs.0.08, p < 0.05),且總分亦於PCI後三個月顯著高於PCI後立即的得分(0.10 vs. 0.03, p < 0.05)。研究中探索影響PCI後三個月全身出血傾向的預測因子,在控制相關變項後,發現服用健康食品為顯著的預測因子(F=15.36,p<0.0001)解釋力達14.1%。另外研究發現且無論服用1種、2種或3種(含)以上,三個月後的出血傾向皆高於三個月前,且服用健康食品種類越多,出血的分越高(F=4.317,p=0.02),其中服用三種(含)以上健康食品出血平均得分高於服用一種健康食品組出血平均得分。
總結,PCI後三個月,服用健康食品是全身出血傾向的唯一預測因子。另外發現服用3種(含)以上健康食品增加出血風險。因此在預行PCI患者可多了解個案是否服用健康食品,進而密切觀察出血情形。
