Abstract
研究背景
甲狀腺功能異常是鋰鹽治療常見的副作用,與難治型情緒障礙相關,目前並無相預測模型供臨床使用。此外,機器學習模型常因特徵維度過高進而影響模型可解釋性與實用性。因此,建立一個具高度透明度與臨床實用性,且同時保有良好預測能力的簡化模型,具有重要價值。

研究目的
本研究旨在運用SHAP進行特徵選擇,簡化鋰鹽相關甲狀腺功能異常的機器學習模型,以提升其臨床適用性。

研究方法
本研究為多中心回溯性世代研究,納入2010至2021期間使用鋰鹽之病人。三家醫院的資料用於模型訓練與內部驗證,另以一所獨立醫院進行外部驗證。首先以遞迴式特徵消除法進行初步特徵選擇,接著使用隨機森林、極端梯度提升、支持向量機與羅吉斯回歸建構模型。表現最佳之模型會進一步以SHAP特徵降維建立簡化模型,最終透過比較模型的預測效能、臨床適用性與SHAP解釋結果評估簡化模型的臨床可用性。

研究結果
本研究納入的1,595名病人中有113人發生甲狀腺不良事件。極端梯度提升模型在四種模型中表現最佳,其外部驗證AUROC與AUPRC為0.773與0.444。SHAP降維後的簡化模型AUROC則為0.803,AUPRC為0.489,與原始模型無顯著統計差異。SHAP分析顯示兩模型的重要特徵高度重疊,包括劑量變化、累積治療天數、基線總膽固醇與基線促甲狀腺激素。

結論
本研究利用常規臨床資料建立了一個簡化且具臨床實用性的極端梯度提升模型,有效預測鋰鹽相關的甲狀腺功能異常。透過 SHAP值進行特徵選擇,兼顧模型的預測效能與可解釋性。
