Abstract
背景:肥胖是近幾年來最重要的公共衛生議題之一,肥胖的盛行率在許多國家當中都有增加的趨勢,小孩及青少年肥胖所增加的比例更是驚人,過重及肥胖是慢性病主要的危險因子,可能會影響糖尿病、心臟血管疾病與癌症等。另外,不只肥胖可能造成往後慢性病的風險,近幾年來體重過輕的比例也是逐年增加,台灣體重過輕人口比率最高的年齡層最多聚集在12~17歲的青少年,相較其他年齡層,這個年齡層的孩子,更有可能是為了迎合所謂時尚或同儕的眼光而過度減肥或節食,青少年的體重不足可能會增加心理問題,像厭食症、憂鬱症、自殺傾向等。體重對青少年最重要也是最常見的後果,就是青少年的社會心理狀況影響,對青少年有重大影響,儘管過去許多研究著重於青少年體重與心理問題的相關,但鮮少研究針對外顯性的心理問題作探討。本研究想探討青少年體重狀態對於青少年社會外顯心理憤怒及違規行為的影響,以及是否會因為性別的不同而有所差異。

方法:使用台灣都會區兒童及青少年肥胖與代謝症候群之流行病學研究之子計畫調查,作為青少年基本人口學變項、父母相關因子及青少年生活型態變項資料來源,總樣本數共340人,並使用貝克兒童及青少年量表第二版中文版中的憤怒與違規行為分量表來測量,並且以多變項線性迴歸分析研究青少年體重狀態對於社會心理問題憤怒及違規行為的影響,並檢視性別與體重狀態之間是否存在交互作用。

結果:本研究中國中青少年過輕佔6.47%、正常佔62.35%、過重/肥胖則佔31.18%,在控制基本人口學變項及其他變項後,體重狀態與憤怒行為沒有顯著差異;與違規行為方面則有顯著差異,體重過重/肥胖者違規行為分數與正常體重青少年相比,分數顯著較低(β=-4.04,p=0.0005),過輕者則沒有險著差異;體重狀態與性別之間沒有存在交互作用,代表體重狀態對於社會心理因子的影響不因為性別而有所差異。

結論:本研究結果顯示青少年體重對於外顯性的心理問題有影響,體重過重/肥胖的青少年違規行為分數較低較不會表現於外在,容易內化於心理傷害自己,造成往後可能有其他心理疾病像憂鬱焦慮等等問題產生。本研究結果可應用於未來規劃青少年體重與社會心理面向之介入計畫,建議在後續未來研究,可以加入同儕關係以及大範圍的年齡來觀察青少年體重與心理的關係,也可以進一步探討性別可能造成差異的原因及機制。
